CT增强扫描联合CT纹理参数评估结肠癌患者临床分期的价值

Evaluation of Clinical Staging Value of CT Enhanced Scanning Combined with CT Texture Parameters in Colon Cancer Patients

目的分析电子计算机断层成像(CT)增强扫描联合CT纹理参数对结肠癌临床分期的评估价值。方法收集2020年1年至2021年1月经手术病理检查确诊为结肠癌的84例患者的临床资料。所有患者术前均经CT增强扫描检查,采用纹理分析软件提取纹理参数,根据手术病理分期结果,比较不同分期患者纹理参数。绘制受试者工作特征(ROC)曲线,分析CT增强扫描联合CT纹理参数对结肠癌临床分期的评估价值。结果病理结果显示,结肠癌Ⅰ期44例,Ⅱ期20例,Ⅲ期20例。不同分期患者偏度、熵、均值比较,差异有统计学意义(P<0.05);另比较峰度,差异无统计学意义(P>0.05);ROC曲线显示,偏度、熵、均值对结肠癌临床分期具有一定的评估价值(AUC>0.70),且三者联合具有较高的评估价值(AUC>0.90)。结论CT增强扫描联合纹理参数对结肠癌临床分期具有较高的评估价值。

Objective To analyze the clinical staging of colon cancer with enhanced CT scanning and CT texture parameters.Methods The clinical data of 84 patients with colon cancer diagnosed by surgery and pathology from January 2020 to January 2021 were analyzed.All patients were examined by enhanced CT scanning before surgery.Texture parameters were extracted by texture analysis software.According to the staging results of surgical pathological results,the texture parameters of patients with different stages were compared.The receiver operating characteristic(ROC)curve was drawn to analyze the evaluation value of enhanced CT scanning combined with CT texture parameters in the clinical staging of colon cancer.Results Pathological results showed that there were 44 cases of stage I colon cancer,20 cases of stage Ⅱ colon cancer and 20 cases of Stage Ⅲ.There were statistical significant differences in skewness,entropy and mean value among patients in different stages(P<0.05).There was no statistical difference in kurtosis(P>0.05).The ROC curve shows that skewness,entropy and mean have certain evaluation value for clinical staging of colon cancer(AUC>0.70),and the combination of the three has high evaluation value(AUC>0.90).Conclusion Enhanced CT scanning combined with texture parameters has a high evaluation value for clinical staging of colon cancer.
