Effect of Quality Care Service Aimed at Promoting Role Adaptation on the Intervention of Exclusive Breastfeeding Rate,Psychological Resilience and Subjective Well-Being of Senior Full-Term Primiparous Mothers

目的探讨以促进角色适应为目标的优质护理服务对高龄足月初产妇纯母乳喂养率、心理弹性及主观幸福感的影响。方法选取2021年1月至2022年10月于河南中医药大学第三附属医院分娩的91例高龄足月初产妇,按随机数字表法分为两组,对照组45例产妇接受产科常规护理,于此基础上观察组46例产妇接受以促进角色适应为目标的优质护理服务。观察两组不同时间点喂养方式、心理弹性量表(CD-RISC)、自我效能量表(BESE)及总体幸福感量表(GWBS)。结果产后1个月、3个月,观察组纯母乳喂养率高于对照组(P<0.05);产后3个月观察组CD-RISC量表力量性、坚韧性、乐观性维度评分均高于对照组(P<0.05);产后3个月观察组BESE量表内心活动及技能维度评分高于对照组(P<0.05);产后3个月观察组GWBS量表总评分高于对照组(P<0.05)。结论以促进角色适应为目标的优质护理服务能增强产妇心理弹性,提高其主观幸福感及自我效能感,能进一步提高产妇纯母乳喂养率。

Objective To explore the impact of quality care services aimed at promoting role adaptation on the rate of exclusive breastfeeding,psychological resilience and subjective well-being of elderly full-term primiparous mothers.Methods A total of 91 cases of elderly full-term primiparas who delivered in the Third Affiliated Hospital Of Henan University Of Traditional Chinese Medicine from January 2021 to October 2022 were selected and randomly divided into two groups.Forty-five cases in the control group were given routine obstetric care,while 46 cases in the observation group were given high-quality nursing service aimed at promoting role adaptation.The feeding patterns,Connor-Davidson resilience scale(CD-RISC),breastfeeding self efficacy scale(BESE)and general well-being schedule(GWBS)were observed at different time points in the two groups.Results At 1 month and 3 months after delivery,the exclusive breastfeeding rate in the observation group was higher than those in the control group(P<0.05).The score of strength,tenacity and optimism of CD-RISC scale in the observation group was higher than that in the control group at 3 months postpartum(P<0.05).The score of inner activity and skill on BESE scale in the observation group was higher than that in the control group at 3 months postpartum(P<0.05).The total score of GWBS in the observation group was higher than that in the control group at 3 months after delivery(P<0.05).Conclusion High quality nursing service aimed at promoting role adaptation can enhance maternal psychological resilience,improve their subjective well-being and self-efficacy,and further improve the rate of exclusive breastfeeding.
