摘要
目的探讨集成磁共振定量参数对移行带前列腺癌(PCa)和前列腺增生(BPH)的诊断意义。方法前瞻性收集2021年4月至2022年4月于本院接受前列腺检查的临床可疑BPH和PCa患者56例,其病变均位于前列腺移行带,其中BPH 36例、PCa 20例,所有患者均行常规序列和集成磁共振成像检查。对前列腺癌(PCa)、间质增生结节(SH)、腺体增生结节(GH)3种病理组织类型分别测量T1、T2、质子密度(PD)值进行比较,组间比较采用单因素方差分析或Kruskal-Wallis H检验,事后两两比较应用Bonferroni法。结果移行带PCa病灶、SH结节和GH结节间比较,T1、T2、PD值总体差异均有统计学意义(P<0.05),GH的三者水平均明显高于PCa和SH,差异有统计学意义(P<0.001)。PCa和SH的T1、T2、PD值差异无统计学意义(P=0.221、1.000、1.000),但PCa的T1值高于SH,二者之间仍存在一定的差异(P=0.221)。结论集成磁共振定量参数对移行带前列腺癌和前列腺增生具有鉴别意义。
Objective To investigate the diagnostic significance of quantitative parameters ofsynthetic MRI in transitional zone prostate cancer(PCa)and prostate hyperplasia(BPH).Methods Prospective collection of patients with clinically suspected BPH and PCa who underwent prostate examination in our hospital from April 2021 to April 2022,All 56 cases were located in the transitional zone of the prostate,36 cases of BPH,20 cases of PCa.All patients underwent conventional sequence and synthetic MRI.The T1,T2,and proton density(PD)values were measured respectively for PCa,stromal hyperplasia(SH),glandular hyperplasia(GH).One--way analysis of variance or Kruskal-Wallis H test was used for the comparison between groups,and Bonferroni method was used for post hoc pairwise comparison.Results Comparison among PCa lesions,SH nodules and GH nodules in transitional zone,The overall difference of T1,T2 and PD values is statistically significant(P<0.05),The three levels of GH were significantly higher than those of PCa and SH,with significant statistical differences(P<0.001).Although there is no significant statistical difference in T1,T2 and PD between PCa and SH(P=0.221、1.000、1.000),but the T1 value of PCa is higher than that of SH,and there is still some difference between them(P=0.221).Conclusion The quantitative parameters of synthetic MRI has discriminates significance in transitional zone PCa and BPH.
