聪脑通络法联合补中益气汤药浴对痉挛型脑瘫患儿的影响

Application of Needling Method of Congnao Tongluo Combined with Medicated Bath of Buzhong Yiqi Tang in Children with Spastic Cerebral Palsy

目的:观察聪脑通络法联合补中益气汤药浴对痉挛型脑瘫患儿的影响。方法:将84例痉挛型脑瘫患儿按随机数字表法分为对照组和观察组各42例。对照组予补中益气汤药浴,观察组在对照组基础予聪脑通络法针刺。4周为1个疗程,治疗3个疗程后比较两组患儿治疗前后粗大运动功能(gross motor function measure,GMFM)量表、精细运动功能(fine motor function measure,FMFM)量表、临床痉挛指数(clinical spasticity index,CSI)量表、Ashwonh肌张力评定量表评分及大脑前、中、后动脉的平均血流速度与临床疗效。结果:观察组总有效率为92.86%(39/42),高于对照组的76.19%(32/42)(P<0.05)。观察组治疗后GMFM、FMFM评分升高程度较对照组明显,CSI、Ashworth评分降低程度较对照组明显,大脑前、中、后动脉血流速度加快程度较对照组明显,差异均有统计学意义(P<0.05)。结论:聪脑通络法联合补中益气汤药浴可改善痉挛性脑瘫患儿粗大运动功能、精细运动功能及肌力,缓解肌肉痉挛,增加脑血流速度,联合治疗效果优于单纯补中益气汤药浴。

Objective:To discuss the impacts of needling method of Congnao Tongluo and medicated bath of Buzhong Yiqi Tang on children with spastic cerebral palsy(SCP).Methods:All 84 children were allocated to the control group and the observation group according to random number table method with 42 cases in each group.The control group accepted medicated bath of Buzhong Yiqi Tang,and the observation group was treated by needling method of Congnao Tongluo based on the therapy the control group accepted.Four weeks were one course of the treatment,to observe gross motor function measure(GMFM),fine motor function measure(FMFM)scales,clinical spasticity index(CSI)scales,Ashwonh scales,mean blood flow velocity in anterior,middle and posterior cerebral arteries and curative effects before and after the treatment between both groups when they were treated for three courses of the treatment.Results:Total effective rate of the observation group was 92.86%(39/42),higher than 76.19%(32/34)of the control group(P<0.05).The scores of GMFM and FMFM of the observation group were increased more apparently than these of the control group,the decrease of CSI and Ashworth scores were more obvious than these the control group,blood flow velocities in anterior,middle and posterior cerebral arteries were improved than these of the control group,and the difference had statistical meaning(P<0.05).Conclusion:Needling method of Congnao Tongluo and medicated bath of Buzhong Yiqi Tang could improve children's GMFM,FMFM and muscle force,relieve muscle spasm,raise cerebral blood flow velocity,and its effects are superior to these of medicated bath of Buzhong Yiqi Tang.
