摘要
目的分析细菌耐药率与抗菌药物使用强度之间的相关性。方法使用WHONET 5.6软件分析2017至2021年临床送检细菌标本的耐药率,统计同期抗菌药物使用强度,对细菌耐药率与抗菌药物使用强度进行相关性分析。结果2017至2021年检出率较高的病原菌依次为:革兰阴性杆菌中的大肠埃希菌、肺炎克雷伯菌,铜绿假单胞菌和鲍曼不动杆菌;革兰阳性球菌中的金黄色葡萄球菌、肺炎链球菌和屎肠球菌。鲍曼不动杆菌的耐药率与亚胺培南的抗菌药物使用强度(AUD)呈正相关(P<0.05)。革兰阳性球菌耐药率与抗菌药物使用强度未见相关性(P>0.05)。结论该院革兰阴性菌耐药率与抗菌药物使用强度之间存在一定的相关性,应加强院内抗菌药物的管理以减缓耐药菌产生。
AIM To analyze the correlation between bacterial resistance rate and antibiotics use density(AUD)in a general hospital.METHODS Drug resistance rate of bacteria isolated from clinical samples from 2017 to 2021 were analyzed with WHONET5.6 software.Concurrently,the antibacterial use density(AUD)of antibiotics during the same period was statistically analyzed.Correlation analysis was performed to assess the relationship between bacterial drug resistance rate and AUD.RESULTS The pathogenic bacteria with higher detection rate from 2017 to 2021 were as follows:Escherichia coli and Klebsiella pneumoniae,Pseudomonas aeruginosa and Acinetobacter Bowman in gramnegative bacteria.Staphylococcus aureus,Streptococcus pneumoniae,Enterococcus faecium in gram-positive bacteria.The resistance rate of Acinetobacter Bowman was positively correlated with AUD of imipenem(P<0.05).No correlation was observed between the resistance rate of gram-positive bacteria and antibiotics use density(P>0.05).CONCLUSION The hospital exhibits a certain correlation between drug resistance rate of gram-negative bacteria and antibiotics use density.Therefore,strengthening the management of antimicrobial drugs within the hospital is advisable to mitigate the emergence of drug-resistant strains.
