摘要
目的观察回阳救心方在脓毒症心功能障碍(SIMD)患者的疗效及对炎症反应的控制效果。方法将2022年3月至2023年5月杭州市第一人民医院81例SIMD患者按治疗方法分为2组,其中对照组采用西医常规方案,观察组基于西医常规方案的同时给予回阳救心方治疗,比较心肌损伤标志物、心脏超声、炎症因子、乳酸、急性生理和慢性健康评分(APACHEⅡ)、脓毒症相关序贯器官衰竭(SOFA)评分和中医证候评分和疗效。结果相较于对照组,观察组患者的心肌损伤标志物(肌钙蛋白、肌酸激酶同工酶、N-末端脑钠肽前体)和炎症指标(C反应蛋白、白细胞介素6、降钙素原)水平更低(P<0.05),左室射血分数和心脏每搏输出量(SV)更高(P<0.05),APACHEⅡ评分、SOFA评分和中医证候评分更低(P<0.05),疗效更好(P<0.05)。结论在西医综合治疗的基础上使用回阳救心方,可有效减轻SIMD患者炎症反应,改善心功能状况和预后。
Objective To observe the therapeutic effect of Huiyang Jiuxin formula in patients with sepsis-induced myocardial dysfunction(SIMD)and its control effect on inflammation.Methods A total of 81 patients with SIMD who attended Hangzhou First Pepole′s Hospital from March 2022 to May 2023 were divided into two groups using random number table method.The control group was treated with conventional western medicine,and the observation group was treated with Huiyang Jiuxin formula and conven-tional Western treatment.Myocardial injury markers,cardiac ultrasound,inflammatory factors,lactic acid,A-PACHEⅡscore,SOFA score,TCM(Tra-ditional Chinese Medicine)syndrome score,and efficacy were compared.Results Compared with the control group,the levels of myocardial injury markers(cTnI,CK-MB and NT-proBNP)and inflammatory indicators(hs-CRP,PCT and IL-6)were lower in the observation group(P<0.05),and the levels of LVEF and SV were higher(P<0.05).APACHEⅡscore,SOFA score and TCM syndrome score were lower(P<0.05),and the curative effect was better(P<0.05).Conclusion Huiyang Jiuxin formula can effectively reduce inflammation and improve cardi-ac function and prognosis in patients with SIMD on the basis of Western comprehensive treatment.
