摘要
目的探讨不同入路导管接触性溶栓(CDT)治疗下肢深静脉血栓(DVT)的疗效。方法收集2021年1月至2023年1月于内蒙古自治区人民医院就诊的92例急性下肢DVT患者的临床资料,按照入路方式的不同将其分为观察组(n=46,采用患肢腘静脉入路置入溶栓导管)和对照组(n=46,健侧股静脉穿刺入路置入溶栓导管),比较两组患者患肢消肿率、静脉通畅率、并发症发生情况。结果观察组患者消肿率、静脉通畅率分别为(79.05±6.69)%、(70.96±12.13)%,均高于对照组患者的(52.70±6.35)%、(55.33±18.48)%(P﹤0.05)。两组患者并发症总发生率比较,差异无统计学意义(P﹥0.05)。结论腘静脉入路置管CDT治疗下肢DVT疗效优于健侧股静脉入路置管,其静脉瓣膜损伤少,溶栓效率高,安全可靠,值得在临床中推广应用。
Objective To investigate the efficacy of catheter-directed thrombolysis(CDT)with different approaches in the treatment of lower extremity deep vein thrombosis(DVT).Method Clinical data of 92 patients with lower extremity DVT who were admitted to the Inner Mongolia People's Hospital from January 2021 to January 2023 were collected,they were divided into observation group(n=46,given antegrade popliteal vein catheterization)and control group(n=46,given contralateral retrograde femoral vein catheterization).The detumescence rate,venous patency rate and complications were compared between the two groups.Result The detumescence rate and patency rate of the observationl group were(79.05±6.69)%and(70.96±12.13)%,respectively,significantly higher than those of the control group(52.70±6.35)%and(55.33±18.48)%(P<0.05).The total incidence of complications in the two groups were not statistically significant difference(P>0.05).Conclusion The antegrade popliteal vein catheterization is more effective than the contralateral retrograde femoral vein catheterization in the treatment of lower extremity DVT,which has less venous valve injury and high thrombolysis efficiency,it is safe and worthy of promotion in clinical practice.
