参芪地黄汤加减治疗早期糖尿病肾病的临床效果

Clinical Effect of Shenqi Dihuang Decoction in the Treatment of Early Diabetes Nephropathy

目的分析在早期糖尿病肾病的治疗中应用参芪地黄汤加减治疗的临床效果。方法抽取2021年5月—2022年5月就诊宁德市中医院肾病科的80例早期糖尿病肾病患者。以抛硬币法分组,各40例。参比组采用常规西药治疗,分析组在此基础上联合参芪地黄汤加减治疗。比较2组治疗前后中医症候积分、血糖指标及肾功能指标。结果2组治疗后中医证候积分(神疲乏力、口干舌燥、腰膝酸软、自汗盗汗、口唇紫黯及舌胖齿痕)低于治疗前,且分析组中医证候积分低于参比组,差异有统计学意义(P<0.05)。2组治疗后血糖指标[空腹血糖(fasting blood glucose,FPG)、餐后2 h血糖(2-hour postprandial blood glucose,2 hPG)、糖化血红蛋白(glycosylated hemoglobin,Hb Alc)]低于治疗前,且分析组血糖指标低于参比组,差异有统计学意义(P<0.05)。2组治疗后肾功能指标均优于治疗前;分析组治疗后血肌酐(serum creatinine,Scr)、尿素氮(blood urea nitrogen,BUN)、尿白蛋白肌酐比值(urinary albumin to creatinine ratio,UACR)水平分别为(72.13±4.12)μmol/L、(4.22±0.44)mmol/L、(57.46±4.98)μg/min,低于参比组的(76.50±4.91)μmol/L、(5.20±0.41)mmol/L、(65.77±4.12)μg/min,差异有统计学意义(P<0.05)。结论早期糖尿病肾病的治疗中应用参芪地黄汤加减治疗有利于缓解患者临床症状,同时还可有效控制患者肾功能及血糖水平。

Objective To analyze the clinical effect of Shenqi Dihuang decoction in the treatment of early diabetes nephropathy.Methods A total of 80 patients with early diabetic nephropathy were selected from department of nephrology,Traditional Chinese Medicine Hospital of Ningde from May 2021 to May 2022.Groups were divided into 40 cases by coin toss.The reference group was treated with conventional western medicine,and the analysis group was treated with Shenqi Dihuang decoction on this basis.TCM syndrome score,blood glucose index and renal function index were compared between the two groups before and after treatment.Results The TCM syndrome scores(fatigue and fatigue,dry mouth and tongue,soreness and weakness in the waist and knees,self sweating and night sweating,purple and dark lips,and fat and dentate marks on the tongue)of the 2 groups after treatment were lower than before treatment,and the TCM syndrome scores of the analysis group were lower than those of the reference group,with statistical significance(P<0.05).The blood glucose indexes[fasting blood glucose(FPG),2-hour postprandial blood glucose(2 hPG)and glycosylated hemoglobin(HbAlc)]of the 2 groups after treatment were lower than before treatment,and the blood glucose indexes in analysis group were lower than those in reference group,with statistical significance(P<0.05).The renal function indexes of 2 groups after treatment were better than before treatment.After treatment,the levels of serum creatinine(Scr),blood urea nitrogen(BUN),urinary albumin to creatinine ratio(UACR)in the analysis group were(72.13±4.12)μmol/L,(4.22±0.44)mmol/L and(57.46±4.98)μg/min,respectively.Lower than(76.50±4.91)μmol/L,(5.20±0.41)mmol/L,(65.77±4.12)μg/min in reference group,the difference was statistically significant(P<0.05).Conclusion In the treatment of early diabetes nephropathy,the application of Shenqi Dihuang decoction can alleviate the clinical symptoms of patients,and can also effectively control the renal function and blood sugar level of patients.
