中西医结合治疗糖尿病下肢血管病变临床效果的meta分析

Meta-Analysis of the Clinical Efficacy of Integrated Traditional Chinese and Western Medicine in the Treatment of Diabetic Lower Extremity Vascular Disease

目的对中西医结合治疗糖尿病下肢血管病变的临床疗效进行系统评价。方法计算机检索CNKI、CBM、万方、VIP等数据库,搜集中西医结合治疗糖尿病下肢血管病变的临床随机对照试验。按照Cochrane偏倚风险评价对纳入文献进行质量分析,采用Revman 5.3以及Stata 16软件进行数据分析。结果本次研究共纳入12篇文献,共1237例患者,其中对照组617例接受西医常规治疗,试验组620例在西医常规治疗的基础上加用中药治疗,meta分析结果显示中西医结合治疗总有效率与单纯西医治疗相比,差异有统计学意义[RR=1.23(1.17,1.29),I^(2)=0,P<0.01],中西医结合治疗后踝肱指数提高优于单纯西医治疗[MD=0.11(0.09,0.13),I^(2)=18%,P<0.01]。结论在治疗糖尿病下肢血管病变方面,中西医结合治疗的临床总有效率和踝肱指数均优于单纯西医治疗。

Objective To systematically evaluate the clinical efficacy of integrated traditional Chinese and Western medicine in the treatment of diabetic lower extremity vascular disease.Methods CNKI,CBM,Wanfang,VIP and other databases were searched by computer to collect clinical randomized controlled trials of integrated traditional Chinese and Western medicine in the treatment of diabetic lower extremity vascular disease.According to the Cochrane bias risk assessment,the quality of the literature was analyzed,and Revman 5.3 and Stata 16 software were used for data analysis.Results A total of 12 literatures were included in this study,with a total of 1237 patients,of which 617 patients in the control group were treated with conventional western medicine,and 620 patients in the experimental group were treated with traditional Chinese medicine on the basis of conventional western medicine.The total effective rate was significantly better than that of western medicine alone[RR=1.23(1.17,1.29),I^(2)=0,P<0.01],and the improvement of ankle-brachial index after integrated traditional Chinese and western medicine was better than that of western medicine alone[MD=0.11(0.09,0.13),I^(2)=18%,P<0.01].Conclusion The total clinical effective rate and ankle-brachial index index of integrated traditional Chinese and western medicine in the treatment of diabetic lower extremity vascular disease are better than those of simple western medicine treatment.
