红外荧光显像技术联合纳米炭在甲状腺癌根治术中识别甲状旁腺的应用效果

摘要 目的探讨红外荧光显像技术联合纳米炭在甲状腺癌根治术中识别甲状旁腺的应用效果。方法选取我院2021年6月至2022年2月收治的120例甲状腺癌患者为研究对象,采用随机数字表法将其分为对照组和观察组,各60例。两组患者均行双侧甲状腺全切除术+双侧中央区淋巴结清扫和/或侧颈淋巴结清扫术,同时对照组常规应用纳米炭识别甲状旁腺完成手术,观察组应用红外荧光显像技术联合纳米炭识别甲状旁腺完成手术。比较两组的应用效果。结果术后1 d,观察组的血钙、甲状旁腺素(PTH)水平均高于对照组,差异具有统计学意义(P<0.05)。观察组的低钙血症、甲状旁腺功能减退发生率均低于对照组,差异具有统计学意义(P<0.05)。结论红外荧光显像技术应用于甲状腺癌根治术中可辅助识别甲状旁腺,联合纳米炭负显影可弥补纳米炭不能使甲状旁腺与脂肪、胸腺等未黑染组织相区别的不足,从而有效保护甲状旁腺功能,减少并发症发生。 Objective To explore the application effect of infrared fluorescence imaging technology combined with nano-carbon in identifying parathyroid glands in radical thyroidectomy.Methods A total of 120 patients with thyroid cancer admitted in our hospital from June 2021 to February 2022 were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 60 cases in each group.Both groups of patients underwent bilateral total thyroidectomy+bilateral central lymph node dissection and/or lateral cervical lymph node dissection.At the same time,the control group routinely used nano-carbon to identify parathyroid glands to complete the operation,and the observation group used infrared fluorescence imaging technology combined with nano-carbon to identify parathyroid glands to complete the operation.The application effects of the two groups were compared.Results At 1 d after operation,the levels of serum calcium and parathyroid hormone(PTH)in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The incidence of hypocalcemia and parathyroid glands hypofunction in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The infrared fluorescence imaging technology applied in radical thyroidectomy can assist in identifying parathyroid glands,combined with nano-carbon negative imaging,it can make up for the deficiency that nano-carbon can not distinguish parathyroid glands from non-black labeled fat,thymus and other tissues,so as to effectively protect parathyroid function and reduce complications.
