常规取穴配伍八脉交会穴治疗腰椎间盘突出症浅析

A Shallow Analysis of the Treatment of Lumbar Disc Herniation withConventional Selection Combined Compatibility ofEight-Meridian Crossing Points

腰椎间盘突出症主要以腰部疼痛伴下肢后、外侧疼痛、麻木、不适为主症,从经络循行上来看,主要与足太阳膀胱经、足少阳胆经相关,故常规针灸治疗多以局部取穴、循经取穴为主,选取足太阳膀胱经、足少阳胆经腧穴,或是利用奇穴的特殊作用使用单穴治疗。由于腰椎间盘突出症往往涉及腰部及下肢症状,病变范围广泛,除了与以上经脉有关外,还与奇经八脉中的督脉、阳跷脉、阳维脉、带脉相关。基于腰椎间盘突出症临床表现,在常规取穴的基础上,立足经络辨证,依据奇经八脉理论,足太阳经型选取申脉—后溪穴组、足少阳经型选取足临泣—外关穴组配伍治疗,可提高临床疗效。针刺时,强调要先针下肢穴,后取上肢穴。故笔者在临床针刺申脉—后溪穴组时要先取申脉,后取后溪;针刺足临泣—外关穴组时要先取足临泣,后取外关。针刺时宜先针刺八脉交会穴,后针刺常规穴。

Lumbar disc herniation is mainly characterized by lumbar pain with posterior and lateral pain,numbness and discomfort of the lower limbs.From the perspective of meridian circulation,it is mainly related to the bladder meridian of foot greater yang and the gall-bladder meridian of foot lesser yang.Therefore,conventional acupuncture treatment is mainly based on local acupoint selection and me-ridian-based acupoint selection,which selects the bladder meridian of foot greater yang and the gallbladder meridian of foot lesser yang,or uses the special function of some extraordinary acupoints.Because lumbar disc herniation often involves symptoms of the waist and lower limbs,and the lesion range is wide,in addition to the above meridians,it is also related to govern vessel(GV),yang heel vessel,yang link vessel,and belt vessel in extraordinary eight acupoints.Based on the clinical manifestations of lumbar disc herniation,besides conventional acupoint selection,meridian syndrome differentiation,and the theory of the eight extraordinary meridians are adopted,so Shenmai(BL 62)-Houxi(SI 3)in meridian of foot greater yang,Zuilinqi(GB 41)-Waiguan(SI 3)in meridian of foot lesser yang are selected to improve the clinical efficacy.Acupuncture should be targeted at the lower limbs first,and then the upper limbs.Therefore,in the Shenmai-Houxi compatibility,Shenmai should be selected first in the clinical acupuncture,then Houxi.In the Linqi-Waiguan com-patibilit,Linqi should be selected first in the clinical acupuncture,then Waiguan.Acupuncture should be done first at the eight-meridian crossing acupoints,and then at the conventional acupoints.
