摘要
目的 探讨老年大肠癌患者肿瘤组织中Jagged 2蛋白(JAG2)、抑制素βA(INHBA)、细胞程序性死亡-配体1(PD-L1)表达与预后的关系。方法 选择2017年5月至2019年5月在湖南省肿瘤医院接受手术治疗的112例老年大肠癌患者为研究对象,采用免疫组织化学法检测癌组织和癌旁组织中JAG2、INHBA、PD-L1表达。随访患者的生存时间,Cox回归模型分析JAG2、INHBA、PD-L1表达与老年大肠癌患者预后的关系。结果 患者癌组织中JAG2、INHBA、PD-L1阳性表达率高于癌旁组织,差异有统计学意义(P<0.05)。低分化患者JAG2、INHBA、PD-L1阳性表达率高于中高分化患者,TNM分期为Ⅲ~Ⅳ期患者JAG2、INHBA、PD-L1阳性表达率高于Ⅰ~Ⅱ期患者(P<0.05)。Cox回归模型显示,肿瘤低分化、TNM分期为Ⅲ~Ⅳ期、JAG2、INHBA、PD-L1表达阳性为影响老年大肠癌患者无进展生存时间的独立危险因素(OR>1,P<0.05)。大肠癌患者癌组织中JAG2、INHBA、PD-L1阴性患者总生存率高于JAG2、INHBA、PD-L1阳性患者,差异有统计学意义(P<0.05)。大肠癌患者癌组织中JAG2、INHBA、PD-L1阴性患者无进展生存率高于JAG2阳性患者,差异有统计学意义(P<0.05)。结论 老年大肠癌患者癌组织中JAG2、INHBA、PD-L1阳性表达率较高,且对预后评估具有重要意义。
Objective To investigate the relationship between the expression of Jagged 2 protein(JAG2),inhibinβA(INHBA),programmed cell death-ligand 1(PD-L1)and prognosis in elderly patients with colorectal cancer.MethodsA total of 112 elderly patients with colorectal cancer who received surgical treatment in Hunan Cancer Hospital from May 2017 to May 2019 were selected as the study objects.JAG2,INHBA,and PD-L1 expression in cancer tissues and adjacent tissues were detected by immunohistochemical method.The survival time of patients was followed up,and the relationship between JAG2,INHBA,PD-L1 expression and prognosis of elderly patients with colorectal cancer was analyzed by Cox regression model.Results The positive expression rates of JAG2,INHBA,and PD-L1 in cancer tissues were higher than those in adjacent tissues,and the differences were statistically significant(P<0.05).The positive expression rates of JAG2,INHBA,and PD-L1 in poorly differentiated patients were higher than those in middle and highly differentiated patients,and the positive expression rates of JAG2,INHBA,and PD-L1 in patients with TNM stageⅢ-Ⅳwere higher than those in patients with stageⅠ-Ⅱ(P<0.05).Cox regression model showed that low tumor differentiation,TNM stageⅢ-Ⅳ,JAG2,INHBA,and PD-L1 expression were independent risk factors for progression-free survival in elderly patients with colorectal cancer(OR>1,P<0.05).The overall survival rate of JAG2,INHBA,and PD-L1 negative patients with colorectal cancer were higher than those of JAG2,INHBA,and PD-L1 positive patients,and the differences were statistically significant(P<0.05).The progression-free survival rate of JAG2,INHBA,and PD-L1 negative patients were higher than those of JAG2 positive patients,and the differences were statistically significant(P<0.05).Conclusion The positive expression rates of JAG2,INHBA,and PD-L1 in cancer tissues of elderly patients with colorectal cancer are high,which is of great significance for prognosis evaluation.
