自制田七祛瘀胶囊联合利伐沙班对胫骨平台骨折患者术后凝血功能及并发症的影响

Effect of Self-made Tianqi Quyu Capsules Combined with Rivaroxaban on Coagulation Function and Complications of Patients with Tibial Plateau Fracture after Surgery

目的探讨胫骨平台骨折患者术后采用自制田七祛瘀胶囊联合利伐沙班治疗的效果。方法按照随机数表法将2021年1月至2022年6月南昌市洪都中医院收治的胫骨平台骨折患者80例分为两组,各40例,均接受手术治疗,对照组术后采用利伐沙班治疗,观察组术后采用自制田七祛瘀胶囊联合利伐沙班治疗,均治疗14 d。比较两组凝血功能、膝关节功能及并发症。结果两组术后1 d纤维蛋白原(PT)、活化部分凝血活酶时间(APPT)及D-二聚体(D-D)水平均比术前高,观察组术后7 d及术后14 d的D-D水平均比对照组低,差异有统计学意义(P<0.05);术后7 d、术后14 d时,两组PT及APPT水平比较,差异无统计学意义(P>0.05)。两组治疗后Rasmussen评分均比治疗前高,且观察组比对照组高(P<0.05);观察组术后下肢深静脉血栓(DVT)发生率比对照组低(P<0.05)。结论胫骨平台骨折患者术后采用自制田七祛瘀胶囊联合利伐沙班治疗可改善凝血功能,降低DVT发生风险,提高膝关节功能。

Objective To investigate the effect of self-made tianqi quyu capsules combined with rivaroxaban on patients with tibial plateau fracture after operation.Methods According to the method of random number table,80 patients with tibial plateau fracture admitted to Nanchang Hongdu Hospital of Traditional Chinese Medicine from January 2021 to June 2022 were divided into two groups,40 patients in each group,who received surgical treatment.The control group was treated with rivaroxaban after surgery,and the observation group was treated with self-made tianqi quyu capsules combined with rivaroxaban after surgery,all treated for 14 days.Compare the coagulation function,knee joint function,and complications between the two groups.Results The levels of fibrinogen(PT),activated partial thromboplastin time(APPT),and D-dimer(D-D)in the two groups were higher than before surgery on the 1st day after surgery.The D-D levels in the observation group were lower than those in the control group on the 7th and 14th day after surgery,with statistical differences(P<0.05);at 7 days and 14 days after surgery,there was no statistically significant difference in the levels of PT and APPT between the two groups(P>0.05).After treatment,the Rasmussen scores of both groups were higher than before treatment,and the observation group was higher than the control group(P<0.05);the incidence of postoperative deep vein thrombosis(DVT)in the observation group was lower than that in the control group(P<0.05).Conclusion The combination of self-made tianqi quyu capsules and rivaroxaban can improve the blood coagulation function,reduce the risk of DVT and improve the function of knee joint in patients with tibial plateau fracture after operation.
