子宫渔网状重建缝合术在前置胎盘合并穿透型胎盘植入孕妇剖宫产术中的应用效果

Application Effect of Uterine Fishnet Reconstruction Suture in Cesarean Section for Pregnant Women with Placenta Previa Combined with Penetrative Placenta Implantation

目的:探讨子宫渔网状重建缝合术在前置胎盘合并穿透型胎盘植入孕妇剖宫产术中的应用价值。方法:选取2020年4月—2023年5月于安顺市人民医院行剖宫产术的127例前置胎盘合并穿透型胎盘植入产妇作为研究对象,随机分为研究组(n=56)和对照组(n=71)。研究组采用子宫渔网状重建缝合术,对照组采用传统缝合术。比较两组孕妇术中、术后情况及新生儿情况。结果:研究组术中出血量、术中输血量少于对照组,术中输血率、子宫切除率低于对照组,手术时间长于对照组,差异有统计学意义(P<0.05);两组膀胱损伤率比较,差异无统计学意义(P>0.05)。研究组住院时间短于对照组,住院费用少于对照组,差异有统计学意义(P<0.05);两组术后肠梗阻、切口愈合不良、产褥感染及ICU入住率比较,差异无统计学意义(P>0.05)。两组新生儿出生体质量、新生儿窒息率及NICU入住率比较,差异无统计学意义(P>0.05)。结论:子宫渔网状重建缝合术在前置胎盘合并穿透型胎盘植入孕妇剖宫产术中的应用效果显著,可减少术中出血量、输血量,降低术中输血率、子宫切除率,缩短术后住院时间。

Objective:To investigate the application value of uterine fishnet reconstruction suture in cesarean section for pregnant women with placenta previa combined with penetrative placenta implantation.Methods:From April 2020 to May 2023,127 pregnant women with placenta previa combined with penetrative placenta implantation who underwent cesarean section at People's Hospital of Anshun City were selected as the study subjects.They were randomly divided into the study group(n=56)and the control group(n=71).The study group adopted uterine fishnet reconstruction suture and the control group adopted traditional suture.The intraoperative conditions and postoperative conditions of pregnant women as well as neonatal conditions were compared between two groups.Results:The intraoperative blood loss and intraoperative blood transfusion in the study group were less than those in the control group,and the intraoperative blood transfusion rate and hysterectomy rate in the study group were lower than those in the control group,the operation time in the study group was longer than that in the control group,and the difference was statistically significant(P<0.05).The bladder injury rate between the two groups showed no statistically significant difference(P>0.05).The length of hospital stay in the study group was shorter than that in the control group,the cost of hospitalization in the study group was less than that in the control group,and the difference was statistically significant(P<0.05);There was no significant difference in the postoperative intestinal obstruction,poor incision healing,puerperal infection and ICU admission rate between the two groups(P>0.05).There were no significant differences in neonatal birth weight,neonatal asphyxia rate and NICU admission rate between the two groups(P>0.05).Conclusion:In the cesarean section of pregnant women with placenta previa combined with penetrative placenta implantation,uterine fishnet reconstruction suture has a significant application effect,and can reduce the amount of intraoperative blood loss,intraoperative blood transfusion,decrease the intraoperative blood transfusion rate and hysterectomy rate,and shorten the length of hospital stay after surgery.
