彩色多普勒超声在下肢深静脉血栓诊断中的应用价值

Application Value of Color Doppler Ultrasound in Diagnosis of Deep Venous Thrombosis of Lower Limbs

目的:分析彩色多普勒超声在下肢深静脉血栓诊断中的应用价值。方法:选取2020年1月-2023年3月常州市金坛区中医医院收治的疑似下肢深静脉血栓患者180例为研究对象,患者依次完成X线、彩色多普勒超声、静脉造影检查。以静脉造影结果为“金标准”,比较X线、彩色多普勒超声检查的诊断价值。结果:180例疑似下肢深静脉血栓患者中确诊143例,其中股静脉血栓50例,髂静脉血栓40例,腘静脉血栓30例,胫后静脉血栓23例;急性血栓60例,亚急性血栓50例,慢性血栓33例。彩色多普勒超声检查的准确性、敏感性、特异性高于X线,差异有统计学意义(P<0.05)。彩色多普勒超声的股静脉血栓、髂静脉血栓检出率高于X线,差异有统计学意义(P<0.05)。彩色多普勒超声急性血栓、亚急性血栓、慢性血栓检出率高于X线,差异有统计学意义(P<0.05)。结论:下肢深静脉血栓临床诊断中应用彩色多普勒超声的诊断效能更高,有利于检出不同病变部位、疾病类型血栓。

Objective:To analyze the application value of color Doppler ultrasound in the diagnosis of deep vein thrombosis of lower limbs.Methods:From January 2020 to March 2023,180 patients with suspected deep venous thrombosis of lower limbs in Changzhou City Jintan District Hospital of Traditional Chinese Medicine were selected as the study subjects.The patients successively completed X-ray,color Doppler ultrasound and venography.The venography results were used as the gold standard,and the diagnostic value of X-ray and color Doppler ultrasound was compared.Results:Among the 180 patients with suspected deep vein thrombosis of lower limbs,143 cases were confirmed,including 50 cases of femoral vein thrombosis,40 cases of iliac vein thrombosis,30 cases of popliteal vein thrombosis,and 23 cases of posterior tibial vein thrombosis;there were 60 cases of acute thrombosis,50 cases of subacute thrombosis,and 33 cases of chronic thrombosis.The accuracy,sensitivity and specificity of color Doppler ultrasound were higher than those of X-ray,and the difference was statistically significant(P<0.05).The detection rates of femoral vein thrombosis and iliac vein thrombosis by color Doppler ultrasound were higher than those by X-ray,and the difference was statistically significant(P<0.05).The detection rates of acute thrombosis,subacute thrombosis and chronic thrombosis by color Doppler ultrasound were higher than those by X-ray,and the difference was statistically significant(P<0.05).Conclusion:The diagnostic efficiency of color Doppler ultrasound in the clinical diagnosis of deep venous thrombosis of lower limbs is higher,which is helpful to detect the thrombosis at different lesion sites and of different disease types.
