俯卧位护理在重症肺炎患儿机械通气护理中的应用效果

Application Effect of Prone Position Nursing in Mechanical Ventilation Nursing for Severe Pneumonia Children

目的:分析俯卧位护理在重症肺炎患儿机械通气护理中的应用效果。方法:选取2021年12月-2022年12月无锡市儿童医院收治的行机械通气的重症肺炎患儿100例作为研究对象,采用抽签方式分为研究组和对照组,各50例。对照组采用常规护理,研究组在对照组基础上配合俯卧位护理。比较两组肺功能、家长护理满意度、治疗效果、临床指标。结果:护理后,两组动脉血氧分压、动态肺顺应性高于护理前,且研究组高于对照组,两组动脉血二氧化碳分压低于护理前,且研究组低于对照组,差异有统计学意义(P<0.05)。研究组家长护理满意度高于对照组,差异有统计学意义(P=0.007 6)。研究组治疗总有效率高于对照组,差异有统计学意义(P=0.014 4)。研究组体温恢复至正常时间、儿童重症监护室住院时间、咳嗽停止时间、机械通气时间短于对照组,差异有统计学意义(P<0.000 1)。结论:俯卧位护理在重症肺炎患儿机械通气护理中的应用效果显著,可改善患儿肺功能,提升治疗效果,促使患儿尽快康复,提高家长护理满意度。

Objective:To analyze the application effect of prone position nursing in mechanical ventilation nursing for severe pneumonia children.Methods:A total of 100 mechanically ventilated children with severe pneumonia admitted to Wuxi Children's Hospital from December 2021 to December 2022 were selected as the study subjects.They were divided into a study group and a control group by drawing lots,with 50 cases in each group.The control group received routine nursing,while the study group received prone position nursing on the basis of control group.Lung function,parental satisfaction with nursing,treatment effect and clinical indicators were compared between the two groups.Results:After nursing,the arterial partial pressure of oxygen and dynamic lung compliance in the two groups were higher than those before nursing,and these indexes in the study group were higher than those in the control group,while the arterial partial pressure of carbon dioxide in the two groups was lower than that before nursing,and this index in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).The parental satisfaction with nursing in the study group was higher than that in the control group,and the difference was statistically significant(P=0.0076).The total effective rate of treatment in the study group was higher than that in the control group,and the difference was statistically significant(P=0.0144).The time for returning to normal body temperature,the length of stay in the pediatric intensive care unit,the time of cough stopping,and the time of mechanical ventilation in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.0001).Conclusion:The application effect of prone position nursing in mechanical ventilation nursing of children with severe pneumonia is significant,which can improve the lung function of children,enhance the treatment effect,promote the recovery of children,and improve the satisfaction of parents with nursing.
