家庭医生签约服务在社区高血压患者中的应用效果

Application Effect of Family Doctor Contracting Service in Community Hypertensive Patients

目的:探讨家庭医生签约服务在社区高血压患者中的应用效果。方法:选取2021年10月-2022年10月徐汇区虹梅街道社区卫生服务中心收治的高血压患者90例作为研究对象,采用随机数字表法分为对照组和干预组,各45例。对照组采用常规服务,干预组采用家庭医生签约服务。比较两组干预效果。结果:干预后,两组舒张压、收缩压水平低于干预前,且干预组低于对照组,两组自我护理能力量表、Morisky用药依从性量表、健康状况调查简表评分高于干预前,且干预组高于对照组,差异有统计学意义(P<0.05)。干预组疾病知识掌握情况评分高于对照组,不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:家庭医生签约服务在社区高血压患者中的应用效果显著,可降低血压水平及并发症发生率,提高患者用药依从性、疾病认知度、自我护理水平和生活质量。

Objective:To explore the application effect of family doctor contracting service in community hypertensive patients.Methods:A total of 90 patients with hypertension who were treated in the Xuhui District Hongmei Street Community Health Service Center from October 2021 to October 2022 were selected as the study subjects.The patients were divided into control group and intervention group according to random number table method,with 45 cases in each group.The control group adopted conventional service.The intervention group adopted family doctor contracting service.The intervention effects were compared between two groups.Results:After intervention,the levels of diastolic blood pressure and systolic blood pressure in the two groups were lower than those before intervention,and the levels in the intervention group were lower than those in the control group,while the scores of self-care ability scale,Morisky medication adherence scale,and short form health survey in the two groups were higher than those before intervention,and the scores in the intervention group were higher than those in the control group,and the difference was statistically significant(P<0.05).The score of disease knowledge mastery in the intervention group was higher than that in the control group,and the incidence of adverse reactions in the intervention group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of family doctor contracting service in community hypertensive patients has a significant effect,which can reduce blood pressure levels and complication rate,and improve patients'medication compliance,disease awareness,self-care level and quality of life.
