全血滤除白细胞不畅的原因分析及处理措施探讨

Cause Analysis and Treatment Measures of Poor Leukocyte Removal by Whole Blood Filtration

目的:分析全血滤除白细胞不畅的原因并探讨处理措施。方法:选取2021年1月-2022年12月徐州市中心血站采集的153 602袋全血为研究对象,采用白细胞滤器滤除白细胞,分析滤除白细胞不畅的原因及影响因素。结果:153 602袋全血中,滤除白细胞不畅247袋,不畅率为1.61‰;导致滤除白细胞不畅的主要原因为血液凝块,其次为工作人员操作不当、滤器质量问题、冷凝集。献血者性别、年龄、献血次数、献血量及采血护士工作年限是血液凝块的影响因素(P<0.05)。结论:导致滤除白细胞不畅的主要原因为血液凝块,献血者性别、年龄、献血次数、献血量及采血护士工作年限是标本发生血液凝块的影响因素;临床应采取应对措施,避免滤除白细胞不畅。

Objective:To analyze the causes of poor leukocyte removal by whole blood filtration and to discuss the treatment measures.Methods:A total of 153602 bags of whole blood collected by Xuzhou Central Blood Station from January 2021 to December 2022 were selected as the research objects.Leukocytes were filtered by leukocyte filter.The cause and influencing factors of poor leukocyte removal by filtration were analyzed.Results:In 153602 bags of whole blood,there were 247 bags with poor filtration,with a poor rate of 1.61‰.The main reason for poor leukocyte removal by filtration was blood clot,followed by improper operation by staff,filter quality problems and cold agglutination.The blood donor's gender,age,number of blood donation,amount of blood donated,and working years of blood sampling nurse were the influencing factors of blood clot(P<0.05).Conclusion:The main reason for poor leukocyte removal by filtration is blood clot.The blood donor's gender,age,number of blood donation,amount of blood donated,and working years of blood sampling nurse are the influencing factors of blood clot in specimen.It is necessary to take response measures in clinical practice to avoid poor leukocyte removal by filtration.
