硼替佐米联合沙利度胺与地塞米松治疗老年多发性骨髓瘤的效果分析

Analysis of Effect of Bortezomib Combined with Thalidomide and Dexamethasone in Treatment of Multiple Myeloma in Elderly

目的:分析硼替佐米联合沙利度胺与地塞米松治疗老年多发性骨髓瘤的临床效果。方法:选取2017年1月—2022年6月盐城市大丰人民医院收治的60例老年多发性骨髓瘤患者作为研究对象,采用随机数字表法分为两组,各30例。两组均应用地塞米松治疗,对照组在此础上采用沙利度胺治疗,观察组在对照组基础上联合硼替佐米治疗。比较两组临床效果。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.006)。治疗后,两组血清M蛋白、骨髓浆细胞、β_(2)-微球蛋白水平低于治疗前,且观察组低于对照组,两组白蛋白水平高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。两组各项不良反应发生率比较,差异无统计学意义(P>0.05)。结论:硼替佐米联合沙利度胺与地塞米松治疗老年多发性骨髓瘤的临床效果显著,可改善患者骨髓状况、凝血功能与肾功能,且未增加不良反应发生率。

Objective:To analyze the clinical effect of bortezomib combined with thalidomide and dexamethasone in the treatment of multiple myeloma in the elderly.Methods:Sixty elderly patients with multiple myeloma admitted to Yancheng City Dafeng People's Hospital from January 2017 to June 2022 were selected as the study subjects.They were divided into two groups using a random number table method,with 30 cases in each group.Both groups received dexamethasone treatment.The control group additionally received thalidomide treatment,while the observation group received bortezomib treatment on the basis of the control group.Clinical effects were compared between two groups.Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.006).After treatment,the levels of M-protein,bone marrow plasma cells andβ2-microglobulin in two groups were lower than those before treatment,and the levels in the observation group were lower than those in the control group;the levels of albumin in two groups were higher than those before treatment,and the levels in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The clinical effect of bortezomib combined with thalidomide and dexamethasone in the treatment of elderly multiple myeloma is significant,which can improve bone marrow conditions,coagulation function and renal function without increasing the incidence of adverse reactions.
