茵陈蒿汤加味辅助治疗湿热郁结型妊娠肝内胆汁淤积症的临床效果

Clinical Effect of Modified Yinchenhao Decoction Auxiliary Treatment for Intrahepatic Cholestasis of Pregnancy withDampness Heat Stagnation

目的:观察茵陈蒿汤加味辅助治疗湿热郁结型妊娠肝内胆汁淤积症的临床效果。方法:选取2021年2月—2022年2月湖南省直中医医院收治的湿热郁结型妊娠肝内胆汁淤积症患者70例作为研究对象,采用随机数字表法分成对照组和观察组,各35例。对照组采取常规治疗,观察组在对照组基础上给予茵陈蒿汤加味辅助治疗。比较两组治疗总有效率、中医证候积分、肝功能指标及妊娠结局。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.012)。治疗后,两组身目发黄、皮肤瘙痒、食欲不振积分低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、总胆汁酸水平低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组妊娠不良事件总发生率低于对照组,差异有统计学意义(P=0.036)。结论:茵陈蒿汤加味辅助治疗湿热郁结型妊娠肝内胆汁淤积症的效果显著,可缓解患者症状,改善肝功能,调节胆汁酸水平,降低妊娠不良事件发生率。

Objective:To observe the clinical effect of modified Yinchenhao Decoction auxiliary treatment for intrahepatic cho-lestasis of pregnancy with dampness heat stagnation.Methods:Seventy patients with intrahepatic cholestasis of pregnancy with dampness heat stagnation admitted to Hunan Province Directly Affiliated TCM Hospital from February 2021 to February 2022 were selected as the study subjects.They were divided into a control group and an observation group according to random number table method,with 35 patients in each group.The control group received routine treatment,while the observation group received adjuvant treatment with modified Yinchenhao Decoction on the basis of the control group.The total effective rate of treatment,tra-ditional Chinese medicine syndrome scores,liver function indicators,and pregnancy outcomes were compared between the two groups.Results:The total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.012).After treatment,the scores of yellowing body and eyes,itchy skin and loss of appetite in the two groups were lower than those before treatment,the scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of alanine aminotransferase,aspartate aminotrans-ferase and total bile acids in the two groups were lower than those before treatment,the levels in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The total incidence of pregnancy adverse events in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.036).Conclusion:The modified Yinchenhao Decoction auxiliary treatment for intrahepatic cholestasis of pregnancy with dampness heat stagnation has significant effects,and can relieve the symptoms,ameliorate liver function,regulate the level of bile acid,and re-duce the incidence of pregnancy adverse events.
