痛风患者服药依从性和健康赋权水平及其影响因素分析

State and related factors of medication compliance and health empowerment in gout patients

目的研析痛风患者在痛风相关用药方面的依从性现状以及健康赋权水平,总结相关影响因素,以期为临床提高患者用药依从性提供借鉴。方法采用服药依从性量表(adherence to refills and medications scale)及老年慢性病患者健康赋权量表,采用横断面调查中山大学附属第三医院、中山大学附属第七医院门诊的痛风患者服药依从性(仅痛风相关用药)及健康赋权情况。结果共纳入186例患者,问卷回收有效率为93%。痛风患者年龄范围为25~76岁,平均年龄为(45.8±11.2)岁,其中男性占98.92%。在服药依从性方面,患者评分为(4.32±1.68)分,其中服药依从性差者占81.72%。服药依从性的得分越高,患者自觉健康状况越差,两者呈负相关关系。健康赋权总分平均分为(4.06±0.43)分,患者健康赋权得分与文化程度及自觉健康状况相关。结论在服药方面,痛风患者的依从性较低,分析原因与患者的自觉健康状况存在一定关系。临床医护人员应重视用药健康教育,强调合理且正确的用药意识及信念养成,帮助患者改善服药依从性,保证用药有效性的同时,提高安全性。

Objective To analyze the status quo of medication compliance and health empowerment of gout patients,and summarize related influencing factors,in order to provide reference for improving medication compliance.Methods We used the adherence to medication and health empowerment scale of elderly patients with chronic diseases and the health empowerment scale to investigate the adherence to medication and health empowerment of gout patients in outpatients of the Third Affiliated Hospital of Sun Yat-Sen University and the Seventh Affiliated Hospital of Sun Yat-sen University in a cross-sectional manner.Results A total of 186 patients were included,and the effective rate was 93%.The age of gout patients ranged from 25 to 76 years old,with an average age of(45.8±11.2)years old;among which 98.92%were males.In terms of medication compliance,the score of patients was 4.32±1.68,among which 81.72%had poor medication compliance.The higher the score of medication compliance,the worse the perceived health status of patients,and the two showed a negative correlation.The average total score of health empowerment was 4.06±0.43,and the health empowerment score was related to the educational level and perceived health status of patients.Conclusion In terms of medication,the compliance of gout patients is low,and the analysis of the reason has a certain relationship with the patients′self-conscious health status.Clinical medical staff should pay attention to medication health education,emphasize reasonable and correct medication awareness and belief formation,help patients improve medication compliance,ensure the effectiveness of medication,and improve safety。
