基于循证的集束化护理策略在住院患者静脉血栓栓塞症的研究

Research on evidence-based bundle nursing strategies for inpatients with venous thromboembolism

目的探讨基于循证的集束化护理在住院患者静脉血栓栓塞症(VTE)预防中的应用效果。方法系统检索国内外关于VTE预防的高质量证据,对纳入的文献结合临床背景,经专家小组会议讨论,形成集束化护理预防策略。并以i-PARIHS循证框架为理论框架,将集束化策略应用于临床,评价应用效果。结果集束化护理策略应用后,护士对VTE预防的知信行问卷得分较应用前有所提高,差异有统计学意义(P<0.05)。护士对患者VTE预防执行率高于应用前,差异有统计学意义(P<0.05)。住院患者VTE发生率较应用前明显降低,差异有统计学意义(P<0.05)。结论基于循证的集束化护理策略应用可以提高护士的知信行水平,有效降低住院患者VTE的发生率,值得在临床上推广。

Objective To investigate the application effect of evidence-based bundle nursing strategies in preventing venous thromboembolism(VTE)among inpatients.Methods High-quality evidence related to VTE prevention was systematically retrieved from both domestic and international sources.The selected literature was combined with clinical context and discussed in expert group meetings to develop a bundle nursing prevention strategy.Using the i-PARIHS evidence framework as a theoretical framework,the bundle strategy was applied in clinical settings,and its application effect was evaluated.Results After the application of the bundle nursing strategy,the scores of nurses′knowledge,beliefs,and practices related to VTE prevention were significantly improved compared to before(P<0.05).The nurses′execution rate of VTE prevention measures for patients was also significantly increased after the intervention(P<0.05).The incidence of VTE among inpatients was noticeably decreased after the implementation of the bundle strategy(P<0.05).Conclusion The application of evidence-based bundle nursing strategies can enhance nurses′knowledge,beliefs,and practices related to VTE prevention,leading to a significant reduction in the incidence of VTE among inpatients.This approach holds promise for clinical promotion and implementation.
