凝血联合纤溶指标的检测在妊娠高血压综合征中的诊断价值

The diagnostic value of coagulation combined with fibrinolytic markers in pregnancy induced hypertension syndrome

目的探讨凝血联合纤溶指标检测对妊娠高血压综合征的诊断价值。方法选择2021年1月-2023年2月入院就诊的126例妊娠高血压综合征患者为研究样本,设为观察组,另选择同期入院孕检的正常孕妇126例,设为对照组。两组受检者均接受凝血四项和纤溶四项检测。结果观察组FIB(5.15±1.04)g/L高于对照组的(3.02±0.19)g/L,且PT(10.22±1.18)s、TT(12.19±2.01)s、APTT(23.87±3.03)s低于对照组[PT(12.61±1.29)s、TT(17.14±1.16)s、APTT(30.28±1.72)s],差异显著,P<0.05;观察组DD(17.46±2.93)mg/L、FDP(17.41±2.59)μg/L、PAI-1(58.57±7.53)ng/mL、tPA(4.34±1.66)U/mL均高于对照组[DD(1.32±0.88)mg/L、FDP(2.78±0.22)μg/L、PAI-1(18.65±6.74)ng/mL、tPA(0.41±0.02)U/mL],差异显著,P<0.05。结论凝血和纤溶系统失衡是妊娠高血压综合征患者普遍存在的体征,对凝血和纤溶指标进行联合检测能为妊娠高血压综合征的早期诊断和防治提供依据,保障孕产妇和新生儿的生命安全。

Objective To explore the diagnostic value of coagulation combined with fibrinolytic index detection in pregnancy induced hypertension syndrome.Method 126 patients with gestational hypertension syndrome who were admitted to the hospital from January 2021 to February 2023 were selected as the study sample and set as the observation group.In addition,126 normal pregnant women who were admitted to the hospital during the same period were selected as the control group.Both groups of subjects underwent four tests for coagulation and four tests for fibrinolysis.Result FIB(5.15±1.04)g/L in the observation group was higher than that in the control group(3.02±0.19)g/L,and PT(10.22±1.18)s,TT(12.19±2.01)s,APTT(23.87±3.03)s were lower than those in the control group[PT(12.61±1.29)s,TT(17.14±1.16)s,APTT(30.28±1.72)s],P<0.05;the observation group had higher levels of DD(17.46±2.93)mg/L,FDP(17.41±2.59)μg/L,PAI-1(58.57±7.53)ng/mL,and tPA(4.34±1.66)U/mL than the control group[DD(1.32±0.88)mg/L,FDP(2.78±0.22)μg/L,PAI-1(18.65±6.74)ng/mL,and tPA(0.41±0.02)U/mL],P<0.05.Conclusion Imbalance of coagulation and fibrinolysis systems is a common sign in patients with pregnancy induced hypertension syndrome.Combined detection of coagulation and fibrinolysis indicators can provide a basis for early diagnosis and prevention of pregnancy induced hypertension syndrome,ensuring the safety of pregnant women and newborns.
