表没食子儿茶素没食子酸酯和乳腺癌发病风险的相关性分析

摘要 目的探讨表没食子儿茶素没食子酸酯(EGCG)对湖北省女性乳腺癌发病风险的影响。方法本研究为病例对照研究,选取2019年1月至2022年12月来湖北省妇幼保健院甲状腺乳腺外科就诊的病理学诊断为乳腺癌的986例湖北女性新发患者为观察组,采用1∶1倾向性评分匹配法(PSM),纳入986例在本院接受健康检查乳腺结果为正常的女性作为对照组。采用多因素Logistic回归分析乳腺癌发病和EGCG的相关性,计算比值比(OR)及95%可信区间(CI)。结果多因素Logistic回归分析显示定期摄入EGCG人群的乳腺癌发病风险显著低于未定期摄入EGCG人群(OR=0.702,95%CI:0.571~0.862,P<0.01)。亚组分析显示,在绝经前人群中,定期摄入EGCG(OR=0.328,95%CI:0.244~0.441,P<0.01)显著降低乳腺癌发病风险;而在绝经后人群中,则显著增加乳腺癌发病风险(OR=1.792,95%CI:1.299~2.472,P<0.01)。结论定期摄入EGCG会显著降低湖北省绝经前女性乳腺癌发病的风险,但会显著增加绝经后女性乳腺癌发病风险。 Objective To assess the association of epigallocatechin gallate(EGCG)with the breast cancer risk.Methods In this retrospective case-control study,986 female patients admitted to the Department of Thyroid and Breast Surgery,Maternal and Child Health Hospital of Hubei Province from January 2019 to December 2022 who were newly diagnosed with breast cancer were included as the observation group.Using the 1∶1 Propensity Score Matching(PSM)method,another 986 women with normal mammary gland results who underwent health examination in our hospital were selected as the control group.Multivariate Logistic regression analyses were employed to evaluate the association of EGCG with breast cancer.The odds ratios(OR)and 95%confidence intervals(95%CI)were calculated.Results Multivariate Logistic analysis indicated that the risk of breast cancer was significantly lower in participants with regular intake of EGCG(OR=0.702,95%CI:0.571-0.862,P<0.01).Subgroup analyses showed that regular intake of EGCG was significantly associated with a lower risk for breast cancer in pre-menopausal women(OR=0.328,95%CI:0.244-0.441,P<0.01)but an increased risk in post-menopausal women(OR=1.792,95%CI:1.299-2.472,P<0.01).Conclusion Regular intake of EGCG significantly reduces the risk of breast cancer risk in pre-menopausal women,but increases the risk in post-menopausal women in Hubei Province.Further studies are needed to explore the mechanism.
