围术期快速康复护理模式对老年股骨颈骨折术后谵妄及睡眠质量的影响

Effect of Perioperative Rapid Rehabilitation Nursing Mode on Delirium and Sleep Quality after Neck of Femur Fracture Surgery in the Elderly

目的:探讨围术期快速康复护理模式对老年股骨颈骨折术后谵妄及睡眠质量的影响。方法:选取2020年1月至2021年12月福建省三明市清流县总医院外二科收治的老年股骨颈骨折患者80例作为研究对象,按照随机数字表法随机分为对照组和观察组,每组40例。对照组接受常规护理,观察组接受围术期快速康复护理模式干预,比较2组患者术后谵妄发生率、髋关节功能、睡眠质量等。结果:观察组术后谵妄发生率2.50%低于对照组20.00%(P<0.05);2组术前Harris髋关节功能评分比较无统计学意义(P>0.05),观察组术后1个月、3个月、6个月时Harris髋关节评分高于对照组(均P<0.05);2组术前匹兹堡睡眠质量指数(PSQI)评分比较无统计学意义(P>0.05),观察组术后12 h、24 h、48 h、72 h时PSQI评分低于对照组(均P<0.05)。结论:老年股骨颈骨折患者围术期接受快速康复护理模式干预,可降低术后谵妄发生概率,保证其拥有良好的睡眠质量,更利于髋关节功能恢复。

Objective:To explore the effect of perioperative rapid rehabilitation nursing mode on postoperative delirium and sleep quality of elderly patients with neck of femur fracture.Methods:A total of 80 elderly patients with neck of femur fracture admitted from January 2020 to December 2021 were selected as the research objects,and they were divided into a control group and an observation group by random number table method,with 40 cases in each group.The control group received routine nursing,and the observation group received perioperative rapid rehabilitation nursing intervention.The incidence of postoperative delirium,hip joint function,sleep quality,etc.of the two groups were compared.Results:The incidence of postoperative delirium in the observation group was 2.50%,lower than that in the control group,which was 20.00%(P<0.05).There was no statistically significant difference in Harris hip joint function scores between the two groups before surgery(P>0.05).It was showed that the Harris hip joint score in the observation group was higher than that in the control group at 1 month,3 months,and 6 months after surgery(P s<0.05).There was no statistically significant difference in the preoperative Pittsburgh Sleep Quality Index(PSQI)scores between the two groups(P>0.05).It was showed that the PSQI scores in the observation group were lower than those in the control group at 12 h,24 h,48 h,and 72 h after surgery(P s<0.05).Conclusion:The elderly patients with neck of femur fracture who received rapid rehabilitation nursing intervention in the perioperative period can reduce the incidence of postoperative delirium,ensure their good sleep quality,and be more conducive to the recovery of hip joint function.
