补阳还五汤加味联合负压封闭引流治疗糖尿病足溃疡气血两虚证49例临床观察

Clinical observation of modified Buyang Huanwu Decoction(补阳还五汤)combined with vacuum sealing drainage in the treatment of 49 cases of diabetic foot ulcer with qi-blood deficiency syndrome

目的观察补阳还五汤加味联合负压封闭引流治疗糖尿病足溃疡气血两虚证的临床疗效。方法将98例糖尿病足溃疡气血两虚证患者采用随机数字表法分为对照组和治疗组,各49例。对照组予负压封闭引流治疗,治疗组在对照组治疗方法的基础上联合补阳还五汤加味口服。治疗6周后比较2组的临床疗效及治疗前后的临床症状评分、糖化血红蛋白水平。结果对照组总有效率为81.63%(40/49),治疗组为95.92%(47/49),2组比较差异有统计学意义(P<0.05);治疗后2组临床症状评分及糖化血红蛋白水平明显降低,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组降低更明显,与对照组治疗后比较差异均有统计学意义(P<0.05)。结论补阳还五汤加味联合负压封闭引流治疗糖尿病足溃疡气血两虚证疗效显著,可明显改善患者的临床症状,降低糖化血红蛋白水平,值得临床推广应用。

Objective To observe the clinical effect of modified Buyang Huanwu Decoction(补阳还五汤)combined with vacuum sealing drainage in the treatment of diabetic foot ulcer with qi-blood deficiency syndrome.Methods 98 patients with qi-blood deficiency syndrome of diabetic foot ulcer were divided into control group and treatment group by random number table,49 cases in each group.The control group was treated with vacuum sealing drainage,and the treatment group was treated with Buyang Huanwu Decoction on the basis of the treatment of the control group.After 6 weeks of treatment,the clinical efficacy,the clinical syndrome score and glycosylated hemoglobin level before and after treatment were compared between the two groups.Results The total effective rate was 81.63%(40/49)in the control group and 95.92%(47/49)in the treatment group,the difference was statistically significant(P<0.05).After treatment,the clinical syndrome scores and glycosylated hemoglobin levels in the 2 groups were significantly reduced,with statistical significance compared with the same group before treatment(P<0.05),and the reduction was more obvious in the treatment group,with statistical significance compared with the control group after treatment(P<0.05).Conclusion Modified Buyang Huanwu Decoction combined with vacuum sealing drainage is effective in the treatment of diabetic foot ulcer with qi-blood deficiency syndrome,which can significantly improve the clinical symptoms of patients and reduce the level of glycosylated hemoglobin,so it is worthy of clinical promotion and application.
