快速无痛飞针法联合自拟固津止遗膏治疗小儿遗尿症肾阳虚证30例临床观察

Clinical observation on 30 cases of kidney-yang deficiency syndrome of enuresis treated by rapid and painless flying needling combined with self-prepared Gujin Zhiyi Gao(固津止遗膏)

目的观察快速无痛飞针法联合自拟固津止遗膏治疗小儿遗尿症肾阳虚证的临床疗效。方法将60例遗尿症肾阳虚证患儿采用简单随机分组法分为对照组和治疗组,各30例。对照组每晚睡前口服醋酸去氨加压素0.2 mg;治疗组给予快速无痛飞针法联合自拟固津止遗膏治疗,2组均连续治疗12周,并随访1年。比较2组近期及远期临床疗效,以及治疗前后唤醒阈、膀胱容量及中医证候积分。结果治疗后及随访1年治疗组膀胱容量均明显增大,治疗后对照组膀胱容量明显增大,与同组治疗前比较差异均有统计学意义(P<0.01),且随访1年治疗组增大更明显,与对照组同期比较差异有统计学意义(P<0.05);治疗后及随访1年2组唤醒阈、中医证候积分明显降低,与同组治疗前比较差异均有统计学意义(P<0.05或P<0.01),且治疗组中医证候积分降低更明显,与对照组同期比较差异均有统计学意义(P<0.05);治疗组近期治愈率为73.4%(22/30),对照组为40.0%(12/30),2组比较差异有统计学意义(P<0.01);治疗组远期治愈率为66.7%(20/30),对照组为33.3%(10/30),2组比较差异有统计学意义(P<0.01)。结论快速无痛飞针法联合自拟固津止遗膏治疗小儿遗尿症肾阳虚证疗效显著,可明显改善患儿临床症状,提高近期及远期治愈率,值得临床推广应用。

Objective To observe the clinical effect of rapid and painless flying needling combined with selfprepared Gujin Zhiyi Gao(固津止遗膏)on kidney-yang deficiency syndrome of enuresis in children.Methods 60 enuresis children with kidney-yang deficiency syndrome were divided into control group and treatment group by simple random grouping method,30 cases in each group.The control group took desmopressin acetate 0.2 mg orally before going to bed every night;the treatment group was treated with rapid and painless flying needling combined with self-prepared Gujin Zhiyi Gao,and both groups were treated for 12 weeks and 1 year follow-up.The short-term and long-term clinical effects,awakening threshold,bladder capacity and TCM symptom score before and after treatment were compared between the two groups.Results After treatment and 1 year follow-up,the bladder volume of the treatment group was significantly increased,and the bladder volume of the control group was significantly increased after treatment,with statistical significance compared with the same group before treatment(P<0.01),and the increase was more obvious in the treatment group after 1 year follow-up,with statistical significance compared with the control group at the same period(P<0.05).After treatment and 1 year follow-up,the awakening threshold and TCM syndrome score of the 2 groups were significantly decreased,and the differences were statistically significant compared with those before treatment(P<0.05 or P<0.01),and the TCM syndrome score of the treatment group was more significantly decreased,and the differences were statistically significant compared with the control group at the same period(P<0.05).The short-term cure rate was 73.4%(22/30)in the treatment group and 40.0%(12/30)in the control group.The long-term cure rate was 66.7%(20/30)in the treatment group and 33.3%(10/30)in the control group,the difference between the two groups was statistically significant(P<0.01).Conclusion Rapid painless flying needling combined with self-prepared Gujin Zhiyi Gao is effective in treating kidney-yang deficiency syndrome of enuresis in children,which can markedy improve the clinical symptoms and increase the short-term and long-term cure rate,and is worthy of clinical application.
