对腰椎间盘射频髓核消融与臭氧消融联合应用后对治疗腰椎疼痛的效果分析

摘要 目的研究腰椎疼痛采取腰椎间盘射频髓核消融与臭氧消融联合诊疗的临床效果。方法选取2020年1月—2022年1月就诊于北京市仁和医院疼痛科的72例腰椎间盘突出症患者为研究对象,采取随机数表法分为对照组和治疗组,每组36例。对照组采用低温等离子射频髓核消融术,治疗组采用低温等离子射频髓核消融术+臭氧消融术,比较两组疗效、腰椎功能、腰腿痛水平、髓核回缩效应参数、炎性因子、生活质量。结果治疗组有效率高于对照组,差异有统计学意义(P<0.05)。治疗组Oswestry功能障碍指数低于对照组,日本骨科学会腰痛评价量表评分高于对照组,差异有统计学意义(P<0.05)。治疗组腰腿疼痛评分低于对照组,差异有统计学意义(P<0.05)。治疗组髓核前后径、横径低于对照组,差异有统计学意义(P<0.05)。治疗组C反应蛋白水平为(3.74±1.12)mg/L、白介素-6水平为(96.96±7.40)ng/L低于对照组的(5.15±1.25)mg/L、(144.34±9.25)ng/L,差异有统计学意义(P<0.05)。治疗组社会/生理功能、一般状况等生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论腰椎间盘突出症采取腰椎间盘射频髓核消融与臭氧消融联合诊疗效果更为显著,可改善机体炎症状态,缓解腰椎疼痛,增强髓核回缩效应,促进腰椎功能恢复,提高生活质量。 Objective To study the clinical effect of combined diagnosis and treatment of lumbar disc radiofrequency nucleus pulposus ablation and ozone ablation for lumbar spine pain.Methods A total of 72 patients with lumbar disc herniation admitted to the pain Department of Beijing Renhe Hospital from January 2020 to January 2022 were selected as the study objects,and they were divided into control group and treatment group by random number table method,with 36 cases in each group.The control group was treated with low temperature plasma radiofrequency ablation of nucleus pulposus,and the treatment group was treated with low temperature plasma radiofrequency ablation of nucleus pulposus+ozone ablation.The efficacy,lumbar function,level of back and leg pain,parameters of nucleus pulposus retract effect,inflammatory factors and quality of life of the two groups were compared.Results The effective rate of treatment group was higher than that of control group,and the difference was statistically significant(P<0.05).The Oswestry disability index of the treatment group was lower than that of the control group,and the score of the low Back Pain Evaluation Scale of the Japanese Orthopaedic Society was higher than that of the control group,and the differences were statistically significant(P<0.05).The score of lumbar and leg pain in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).The anteroposterior diameter and transverse diameter of the nucleus pulposus in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).The level of C-reactive protein(3.74±1.12)mg/L and interleukin-6(96.96±7.40)ng/L in the treatment group were lower than those in the control group(5.15±1.25)mg/L and(144.34±9.25)ng/L,and the differences were statistically significant(P<0.05).The scores of social/physiological function,general status and other quality of life in the treatment group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The combined diagnosis and treatment of lumbar disc herniation with radiofrequency nucleus pulposus ablation and ozone ablation is more effective,which can improve the inflammatory state of the body,relieve lumbar pain,enhance the retraction effect of nucleus pulposus,promote the recovery of lumbar function and improve the quality of life.
