中心静脉压监测在重症休克患者中的应用价值

Clinical Value of Central Venous Pressure Monitoring in Patients with Severe Shock

目的分析中心静脉压监测用于重症休克患者的临床价值。方法回顾性分析2021年1月—2023年12月在我院收治的100例重症休克患者临床资料。按照是否给予中心静脉压监测分为两组,对照组选择无中心静脉压监测的常规重症监护和治疗,观察组选择中心静脉压监测的重症监护和治疗,比较两组的临床治疗效果、病情严重程度改善及血流动力学指标(血压、心率)。结果经重症监测及治疗后,与对照组比,观察组的6 h复苏达标率更高,住院期间死亡率更低,平均住院时间更短(均P<0.05);观察组治疗期间的不同时间点APACHEⅡ评分更低(P<0.05);观察组的血流动力学指标更优(均P<0.05)。结论中心静脉压监测用于重症休克患者的干预效果更好,有效提高6 h复苏达标率,降低死亡率,缩短住院时间,优化血流动力学指标,改善疾病严重程度。

Objective To analyze the clinical value of central venous pressure monitoring for patients with severe shock.Methods The clinical data of 100 cases of patients with severe shock in our hospital from January 2021 to December 2023 were retrospectively analyzed.All subjects were divided into two groups according to whether receiving central venous pressure monitoring or not,the control group was given the routine intensive care and treatment without central venous pressure monitoring,and the observation group was given intensive care and treatment with central venous pressure monitoring.The clinical therapeutic effects,improvement of disease severity and hemodynamic indexes(blood pressure,heart rate)of the two groups were compared.Results After intensive monitoring and treatment,compared with the control group,the observation group had a higher 6-h resuscitation attainment rate,a lower mortality rate during hospitalization,and a shorter average hospital stay(all P<0.05);the observation group had a lower APACHEⅡscore at different time points during treatment(P<0.05);and the observation group had better hemodynamic indexes(all P<0.05).Conclusions The central venous pressure monitoring used in patients with severe shock has better intervention effect,effectively improves the 6-h resuscitation rate,reduces the mortality rate,shortens the hospitalization time,optimizes the hemodynamic indexes,and improves the severity of the disease.
