口服吲哚美辛联合放射治疗对预防肘关节异位骨化切除术后骨化复发的效果

Effect of Oral Indomethacin Combined with Radiotherapy on Preventing Recurrence of Ossification after Ectopic Ossification Excision of Elbow Joint

目的观察局部小剂量单次放射治疗联合口服吲哚美辛对预防肘关节异位骨化切除术后复发的效果。方法回顾性分析在我院2010年1月—2022年12月收治的60例肘关节异位骨化行切除术治疗患者的临床资料。根据治疗方案的不同进行分组,其中以口服吲哚美辛治疗的30例患者为对照组,在此基础上联合局部小剂量单次放射治疗的另30例患者为研究组。比较两组患者在治疗前、后的疼痛评分、Mayo肘关节功能评分、上肢运动功能评分,统计两组术后12个月骨化复发情况。结果在治疗前两组疼痛评分、Mayo肘关节功能评分、上肢运动功能相比,并无统计学差异(均P>0.05)。在药物治疗疗程介绍后两组各评分均较治疗前改善,其中研究组均较对照组改善更佳(均P<0.05)。随访12个月骨化复发率研究组低于对照组(P<0.05)。结论在肘关节异位骨化切除术患者中,以局部小剂量单次放射联合口服吲哚美辛,相比单独药物治疗可进一步减少术后复发,促进术后肘关节功能及上肢运动功能恢复。

Objective To observe the effect of local small-dose single radiation therapy combined with oral indomethacin on the prevention of recurrence after resection of elbow joint heterotopic ossification.Methods The clinical data of 60 patients with heterotopic ossification of the elbow treated by resection admitted in our hospital from January 2010 to December 2022 were retrospectively analyzed.They were grouped according to the different treatment plans,in which 30 patients treated with oral indomethacin were the control group,and another 30 patients treated with local small-dose single radiation therapy were the study group.The pain scores,Mayo elbow function scores,and upper limb motor function scores of the two groups were compared before and after the treatment,and the ossification recurrence at 12 months after the operation was counted in the two groups.Results There was no statistical difference in pain scores,Mayo elbow function scores,and upper extremity motor function between the two groups compared before treatment(all P>0.05).After the introduction of the drug treatment course,the scores of both groups improved compared with those before treatment,and the study group improved better than the control group(both P<0.05).The recurrence rate of ossification at 12-month follow-up was lower in the study group than in the control group(P<0.05).Conclusions In patients undergoing resection of heterotopic ossification of the elbow joint,local small-dose single radiation combined with oral indomethacin can further reduce postoperative recurrence and promote postoperative recovery of elbow joint function and upper limb motor function compared with drug treatment alone.
