乳腺纤维腺瘤与脂肪瘤在高频超声下的影像学表现分析

Analysis of imaging findings of breast fibroadenoma and lipoma under high frequency ultrasound

目的探讨乳腺纤维腺瘤与脂肪瘤在高频超声下的影像学表现。方法选取2017年1月至2021年6月于萍乡市人民医院确诊的78例乳腺肿块患者作为研究对象,根据诊断结果分为乳腺纤维腺瘤组(n=66)与乳腺脂肪瘤组(n=12)。所有患者均进行高频超声检查,比较两组高频超声表现、收缩期峰值速度(PSV)、搏动指数(PI)及阻力指数(RI)。结果两组肿瘤边界清晰、圆形或椭圆形、回声均匀、显示血流信号占比比较差异无统计学意义;乳腺纤维腺瘤组包膜回声、位于腺体层及侧壁声影占比均高于乳腺脂肪瘤组,差异有统计学意义(P<0.05)。两组PSV、PI及RI比较差异无统计学意义。结论乳腺纤维腺瘤及脂肪瘤具有不同的高频超声影像学表现,利用高频超声检查可清晰显示肿瘤边界、大小、位置、形态、回声及血流信号等,可为临床鉴别诊断提供一定参考依据。

Objective To investigate the imaging findings of breast fibroadenoma and lipoma under high frequency ultrasound.Methods 78 pa-tients with breast masses diagnosed in Pingxiang People's Hospital from January 2017 to June 2021 were selected as the research subjects,and they were divided into the breast fibroadenoma group(n=66)and the breast lipoma group(n=12)according to the diagnosis results.All patients under-went high frequency ultrasound.The high frequency ultrasound manifestations,peak systolic velocity(PSV),pulsation index(PI)and resistance in-dex(RI)were compared between the two groups.Results There were no significant differences in the proportion of tumors with clear boundary,round or oval shape,uniform echo and blood flow signal between the two groups;the proportion of capsule echo,acoustic shadow located in glandu-lar layer and lateral wall in the breast fibroadenoma group were higher than those in the breast lipoma group,and the differences were statistically sig-nificant(P<0.05).There were no significant differences in PSV,PI and RI between the two groups.Conclusion Breast fibroadenoma and lipoma have different high frequency ultrasound imaging findings,high frequency ultrasound can clearly show the tumor boundary,size,location,shape,echo and blood flow signals,which can provide the reference for clinical differential diagnosis.
