经口腔前庭入路腔镜手术和颈部低位领式切口手术治疗甲状腺良性肿瘤的效果

Effect of Endoscopic Surgery Via Oral Vestibular Approach and Neck Low Collar Incision Surgery in the Treatment of Benign Thyroid Tumors

目的:比较经口腔前庭入路腔镜手术和颈部低位领式切口手术治疗甲状腺良性肿瘤的临床效果。方法:选取2021年1月—2022年10月龙岩市第二医院收治的70例甲状腺良性肿瘤患者,按照随机数表法分为对照组与研究组,各35例。对照组采用颈部低位领式切口手术治疗,研究组则采用经口腔前庭入路腔镜手术治疗。比较两组临床治疗效果、瘢痕外观满意度、围手术期指标、术后疼痛及并发症发生情况等。结果:两组治疗总有效率比较,差异无统计学意义(P>0.05)。研究组切口愈合时间短于对照组,瘢痕宽度小于对照组,瘢痕外观满意度评分高于对照组,差异有统计学意义(P<0.05)。研究组手术时间长于对照组,住院时间短于对照组,术中出血量少于对照组,数字评分量表(NRS)评分低于对照组,差异有统计学意义(P<0.05)。研究组术后并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论:在甲状腺良性肿瘤的临床治疗中,和颈部低位领式切口手术相比,经口腔前庭入路腔镜手术能够显著提高患者瘢痕外观满意度,缓解术后疼痛,同时减少术后并发症的发生。

Objective:To compare the clinical effects of endoscopic surgery via oral vestibular approach and neck low collar incision surgery in the treatment of benign thyroid tumors.Method:A total of 70 patients with benign thyroid tumors admitted to the Second Hospital of Longyan from January 2021 to October 2022 were selected and divided into control group and study group according to random number table method,with 35 patients in each group.The control group was treated with neck low collar incision surgery,and the study group was treated with endoscopic surgery via oral vestibular approach.The clinical treatment effect,scar appearance satisfaction,perioperative indexes,postoperative pain and complications were compared between the two groups.Result:There was no significant difference in the total effective rate between the two groups(P>0.05).The wound healing time of the study group was shorter than that of the control group,the scar width was smaller than that of the control group,and the scar appearance satisfaction score was higher than that of the control group,the differences were statistically significant(P<0.05).The operation time of the study group was longer than that of the control group,the hospitalization time was shorter than that of the control group,the intraoperative blood loss was less than that of the control group,and the numeric rating scale(NRS)score was lower than that of the control group,the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:In the clinical treatment of benign thyroid tumors,compared with neck low collar incision surgery,endoscopic surgery via oral vestibular approach can significantly improve patients'satisfaction with scar appearance,relieve postoperative pain,and reduce postoperative complications.
