炔雌醇环丙孕酮片配合宫腔镜术用于围绝经期异常子宫出血患者的价值

Value of Ethoestradiol Cyproterone Tablets Combined with Hysteroscopy in Perimenopausal Patients with Abnormal Uterine Bleeding

目的:研究炔雌醇环丙孕酮片配合宫腔镜术(HS)用于围绝经期异常子宫出血(AUB)患者的价值。方法:选取2019年6月—2021年6月在将乐县总医院进行治疗的152例围绝经期AUB患者作为研究对象,采用随机数表法分为观察组和对照组,各76例。对照组使用HS治疗,观察组使用炔雌醇环丙孕酮片配合HS治疗。比较两组治疗前后的激素水平[催乳素(PRL)、雌二醇(E_(2))、黄体生成素(LH)、卵泡刺激素(FSH)]、出血停止时间、月经量[月经失血图(PBAC)]、不良反应发生率、血红蛋白浓度及子宫内膜厚度。结果:治疗后,两组PRL、E_(2)、LH、FSH水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组PBAC评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组出血停止时间早于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为3.95%(3/76),低于对照组的5.26%(4/76),但两组间比较,差异无统计学意义(χ^(2)=0.150,P>0.05)。治疗后,两组血红蛋白浓度高于治疗前,子宫内膜厚度低于治疗前,且观察组血红蛋白浓度高于对照组,子宫内膜厚度低于对照组,差异有统计学意义(P<0.05)。结论:采用炔雌醇环丙孕酮片配合HS治疗围绝经期AUB患者能降低其激素水平,缩短止血时间,减少月经量,提高血红蛋白浓度,降低子宫内膜厚度,且较为安全。

Objective:To study the value of Ethoestradiol Cyproterone Tablets combined with hysteroscopy(HS)for perimenopausal patients with abnormal uterine bleeding(AUB).Method:A total of 152 perimenopausal patients with AUB treated in Jiangle General Hospital from June 2019 to June 2021 were selected as the study objects and divided into observation group and control group by random number table method,with 76 cases in each group.The control group was treated with HS,and the observation group was treated with Ethinestradiol Cyproterone Tablets combined with HS.The hormone levels[prolactin(PRL),estradiol(E_(2)),luteinizing hormone(LH),follicle-stimulating hormone(FSH)],bleeding cessation time,menstrual volume[pictorial blood loss assessment chart(PBAC)],incidence of adverse reactions,hemoglobin concentration and endometrial thickness were compared between the two groups before and after treatment.Result:After treatment,the levels of PRL,E_(2),LH and FSH in the two groups were lower than those before treatment,and the levels in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,PBAC scores in both groups were lower than those before treatment,and the score in the observation group was lower than that in control group,the differences were statistically significant(P<0.05).The bleeding cessation time in the observation group was earlier than that in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 3.95%(3/76),which was lower than 5.26%(4/76)in the control group,but there was no significant difference between the two groups(χ^(2)=0.150,P>0.05).After treatment,the hemoglobin concentration in the two groups were higher than those before treatment,and the endometrial thickness were lower than those before treatment,and the hemoglobin concentration in the observation group was higher than that in the control group,and the endometrial thickness was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:The use of Ethinestradiol Cyproterone Tablets combined with HS in the treatment of perimenopausal AUB patients can reduce their hormone levels,shorten the hemostatic time,reduce menstrual volume,increase hemoglobin concentration and reduce endometrial thickness,and is relatively safe.
