1248例呼吸系统疾病患者住院费用关联度分析

Correlation Analysis of Hospitalization Expenses of 1,248 Patients with Respiratory Dis⁃ease in Guizhou Province

目的:分析呼吸系统疾病患者住院费用及影响因素,为优化医疗资源分布、减轻疾病直接经济负担提供参考依据。方法:收集贵州省某三甲医院呼吸内科2022年1 248例呼吸系统疾病患者信息,对住院费用构成进行逐步回归分析,并对费用影响因素进行K-W检验分析。结果:呼吸内科患者住院总费用的中位数为14 461.51元,其中呼吸衰竭患者住院总费用最高,中位数35 293.61元;急性上呼吸道感染费用最低;中位数3 788.63元;不同性别、年龄、住院天数、诊断、医保类型、出院情况的呼吸系统疾病者住院总费用的差异存在统计学意义(P <0.05);对住院总费用影响最大的费用是西药费,其次是治疗费、检查费、护理费及材料费(P <0.05)。结论:呼吸系统患者疾病住院费用带来的经济负担较重,应提高公众预防呼吸系统疾病的意识,可从实施临床路径管理、完善医保政策等方面合理控制疾病直接经济负担。

Objective:To analyze the hospitalization costs and influencing factors of patients with respiratory diseases,and to pro-vide reference for optimizing the distribution of medical resources and reducing the direct economic burden of diseases.Method:The data of 1,248 patients with respiratory diseases in a certain Grade A tertiary hospital in Guizhou Province in 2022 were collected.The factors affecting the total hospitalization expenses were analyzed by Kruskal-Walli rank sum test,and the composition of expenses was analyzed by stepwise regression.Result:The median total hospital expenses of respiratory patients in a third class A hospital in Gui-zhou Province was 14461.51 yuan.Among them,respiratory failure patients had the highest total hospitalization cost,with a median of 35293.61 yuan,acute upper respiratory tract infection cost was the lowest,with a median of 3788.63 yuan.There were statistically sig-nificant differences in the total hospitalization expenses of patients with respiratory diseases with different gender,age,length of stay,diagnosis,type of insurance insurance and discharge condition(P<0.05).The expenses that had the greatest influence on the total hos-pitalization cost were western medicine cost,treatment cost,inspection cost,nursing cost and material cost(P<0.05).Conclusion:The economic burden brought by hospitalization costs of respiratory system in Guizhou is relatively heavy.It is necessary to gradually raise the public awareness of preventing respiratory diseases,and reasonably control the direct economic burden of diseases from the as-pects of implementing clinical pathway management and improving medical insurance policies.
