2017-2022年吉林省某综合医院多重耐药菌监测报告分析

Monitoring Reports Analysis on Multidrug-resistant Bacteria in A Comprehensive Hospital in Jilin Province from 2017 to 2022

摘要 目的 了解本院2017年1月—2022年12月的多重耐药菌监测数据情况并进行分析,为多重耐药菌的临床防控工作提供临床依据。方法 回顾性分析我院的多重耐药菌目标性监测数据,对多重耐药菌的检出情况进行分析。结果 本院2017—2022年多重耐药菌检出率呈逐年上升趋势(P<0.001)。6年间共检出5001株多重耐药菌,其中超广谱β-内酰胺酶(ESBLs)细菌占比最高,检出3703株(74.05%)。多重耐药菌中,MRSA、ESBLs、CRPAE和CRE在2020—2022年的检出数比2017—2019年检出数明显增加,差异有统计学意义(P<0.001)。结论 多重耐药菌的防控工作面临着新的挑战,抗菌药物的广泛使用可能是加剧抗生素耐药危机的重要原因,进而使医院的多重耐药菌感染率上升。面对新的挑战,医院更要不断加强院内感染的多学科联防联控工作,严格进行各项感染的目标性监测工作。 Objective To understand and analyze monitoring data of multidrug-resistant bacteria in our hospital from January 2017 to December 2022,and provide clinical basis for prevention and control of multidrug-resistant bacteria.Methods The paper reviewed and analyzed targeted monitoring data of multidrug-resistant bacteria in our hospital,and analyzed detection of multidrug-resistant bacteria with SPSS 25 statistical tool.Results Detection rate of multidrugresistant bacteria in our hospital showed an increasing trend year by year from 2017 to 2022(P<0.001).Over the past six years,5001 strains of multidrug-resistant bacteria were detected,broad-spectrumβ-Lactamases(ESBLs)bacteria accounted for the highest proportion,3703 strains(74.05%)were detected.Among multidrug-resistant bacteria,detection numbers of MRS,ESBLs,CRPAE,and CRE increased more significantly from 2020 to 2022 than from 2017 to 2019,with statistically significant difference(P<0.001).Conclusion Prevention and control of multidrugresistant bacteria faces new challenges,and widespread use of antibiotics in epidemic era may be an important reason of exacerbating crisis of antibiotic resistance,which causes infection rate increase of multidrug-resistant bacteria in hospitals.Faced new challenges,hospitals should strengthen multidisciplinary joint prevention and control of nosocomial infections continuously,and carry out targeted monitoring of various infections strictly.
