心电散点图在动态心电图心律失常分析中的应用研究

Application Research of Electrocardiogram Scatter plot in Arrhythmias Analysis of Dynamic Electrocardiogram

目的 研究心电散点图用于心律失常患者动态心电图检查分析中的应用价值。方法 选择2022年2月—2023年2月常熟市第二人民医院门诊及住院接诊心律失常患者122例进行回顾性分析,患者均接受24h动态心电图检查,且结合患者症状表现及人工矫正结果作为金标准,绘制心电散点图,根据心电散点图分析诊断心律失常的价值,计算诊断结果与金标准的一致性。计算心电散点图诊断不同类型心律失常的灵敏度、特异性、准确率及Kappa值。结果 122例心律失常患者经金标准诊断结果显示,14例患者诊断为室性心律,36例患者诊断为室性早搏,29例患者诊断为室上性早搏,17例患者诊断为心房颤动,12例患者诊断为心房扑动,8例患者诊断为房颤伴室性早搏,4例患者诊断为房颤伴室内差异性传导,2例患者诊断为房颤合并房扑。根据心电散点图分析结果显示,诊断不同类型心律失常情况的一致性均超过90.00%。通过进一步计算结果显示,心电散点图诊断室性心律、心房扑动等情况的灵敏度、特异度、准确率均为100.00%,诊断室性早搏、室上性早搏、心房扑动的准确率同样高于90.00%。结论 通过心电散点图可用于分析心律失常患者动态心电图诊断结果,具有较高的灵敏度及准确率,能够为临床提供可靠支持,值得推广。

Objective To study application value of electrocardiogram scatter plot in dynamic electrocardiogram examination and analysis of arrhythmia patients.Methods The paper reviewed and analyzed 122 arrhythmia patients admitted to outpatient and inpatient clinics of Changshu City the Second People's Hospital from February 2022 to February 2023,treated with 24-hour dynamic electrocardiogram examination,drew scatter plot of electrocardiogram combined with gold standard of symptoms and artificial correction results of patients,analyzed value of diagnosing arrhythmia based on scatter plot of electrocardiogram,and calculated consistency between diagnostic results and gold standard,and calculated sensitivity,specificity,accuracy,and Kappa value of electrocardiogram scatter plots for diagnosing different types of arrhythmia.Results Gold standard diagnosis of 122 arrhythmia patients showed,14 patients were diagnosed with ventricular rhythm,36 patients with ventricular premature beats,29 patients with supraventricular premature beats,17 patients with atrial fibrillation,12 patients with atrial flutter,8 patients with atrial fibrillation with ventricular premature beats,4 patients with atrial fibrillation with ventricular differential conduction,and 2 patients with atrial fibrillation with atrial flutter.According to analysis results of electrocardiogram scatter plot,consistency in diagnosing different types of arrhythmia exceeded 90.00%.Further calculations showed sensitivity,specificity,and accuracy of electrocardiography were all 100.00%in diagnosing ventricular rhythm,atrial flutter,and other conditions,and accuracy of diagnosing ventricular premature beats,supraventricular premature beats,and atrial flutter was also higher than 90.00%.Conclusion Electrocardiogram scatter plot can analyze diagnostic results of dynamic electrocardiogram of arrhythmia patients,with high sensitivity and accuracy,which can provide reliable support for clinical practice and is worth promoting.
