宫腔声学造影联合三维超声对宫腔粘连的诊断价值分析

Analysis of Diagnostic Value of Uterine Echocardiography Combined with Three-dimensional Ultrasound in Uterine Adhesions

目的 分析在宫腔粘连疾病的临床检查诊断中宫腔声学造影联合三维超声检查的诊断价值。方法 选取2020年12月—2021年12月本院收治的100例疑似宫腔粘连患者作为研究对象,对其开展宫腔声学造影联合三维超声检查后,均进行宫腔镜检查,以宫腔镜检查结果为金标准,观察宫腔声学造影联合三维超声检查结果以及诊断效能,同时分析其联合检查宫腔粘连分度以及宫腔粘连分型和宫腔镜检查结果的一致性。结果100例疑似宫腔粘连患者经宫腔镜检查显示共计91例(91.00%)为宫腔粘连,宫腔声学造影联合三维超声检查结果显示89例(89.00%)为宫腔粘连。宫腔声学造影联合三维超声检查诊断的灵敏度为95.60%(87/91),诊断特异度为77.78%(7/9),诊断准确性为94.00%(94/100)。91例确诊宫腔粘连患者中,对于宫腔声学造影检查和三维超声检查,其联合检查宫腔粘连分度以及宫腔粘连分型的检查结果和宫腔镜检查存在较好的一致性(Kappa=0.791,Kappa=0.797)。结论 在宫腔粘连疾病的临床检查诊断中,宫腔声学造影和三维超声检查具有良好的联合诊断应用价值,可以有效辨别患者宫腔粘连的分度和分型,诊断效能高,值得推广。

Objective To analyze the diagnostic value of contrast-enhanced acoustics combined with three-dimensional ultrasonography in the clinical diagnosis of uterine adhesion diseases.Methods 100 patients with suspected uterine adhesions admitted to our hospital from December 2020 to December 2021 were selected as research objects.Hysteroscopy was performed in all patients after hysteroscopy combined with 3D ultrasound.The results of hysteroscopy combined with 3D ultrasound were regarded as the gold standard to observe the results and diagnostic efficiency of hysteroscopy combined with 3D ultrasound Intrauterine adhesions classification and intrauterine adhesions classification were examined Consistent with the results of hysteroscopy.Results Of 100 suspected patients with intrauterine adhesions,91 cases(91.00%)were found to be intrauterine adhesions by hysteroscopy,and 89 cases(89.00%)were found to be intrauterine adhesions by contrast acoustics combined with three-dimensional ultrasound.The diagnostic sensitivity of contrast-echocardiography combined with 3D ultrasound was 95.60%,(87/91),Diagnostic 77.78%(7/9),the diagnostic accuracy was 94.00%(94/100).Among the 91 patients diagnosed with intrauterine adhesions,the results of the combined examination of intrauterine cavity,adhesiveness grading and intrauterine adhesiveness classification were in good agreement with those of hysteroscopy(Kappa=0.791,Kappa=0.797).Conclusion In the clinical examination and diagnosis of uterine adhesion diseases,the combined diagnosis of uterine contrast echocardiography and three-dimensional ultrasound has good application value,which can effectively distinguish the classification and classification of uterine adhesion of patients,and has high diagnostic efficiency,which is worthy of promotion.
