枸橼酸咖啡因联合无创高频通气治疗新生儿呼吸窘迫综合征的疗效分析

Efficacy Analysis of Caffeine Citrate Combined with Non-invasive High Frequency Ventilation in the Treatment of Neonatal Respiratory Distress Syndrome

摘要 目的 探讨枸橼酸咖啡因联合无创高频振荡通气治疗新生儿呼吸窘迫综合征的疗效分析。方法 选取自2021年10月—2023年3月在本院进行治疗的新生儿呼吸窘迫综合征(NRDS)患儿60例为本次的研究对象,并采用随机数字表法分为对照组及研究组,每组30例。在常规治疗下,对照组给予无创高频振荡通气及补充肺表面活性物质(PS);研究组在对照组治疗基础上添加了枸橼酸咖啡因治疗措施,比较两组临床治疗情况,治疗前后无创通气、常规用氧时间、血气指标及呼吸暂停发生的天数,并记录治疗期间有无并发症。结果 研究组患儿的无创呼吸机辅助应用时间、用氧天数、临床症状缓解时间等均短于对照组,且动脉血气改善程度较对照组高(P<0.05);两组间的并发症总发生率差异无统计学意义(P>0.05)。结论 在无创高频振荡通气联合补充PS治疗NRDS时加用枸橼酸咖啡因,可使患儿无创呼吸机使用天数及用氧时间有效缩短,其临床症状更快得到改善,并降低撤机失败率,且安全性较高。 Objective To investigate the curative effect of caffeine citrate combined with non-invasive high frequency oscillatory ventilation in the treatment of neonatal respiratory distress syndrome.Methods 60 children with NRDS who were treated in our hospital from October 2021 to March 2023 were selected as the study objects,and were divided into two groups by random number table method,including 30 cases in the control group and 30 cases in the study group.Under conventional treatment,the control group was given non-invasive high frequency oscillation and PS application;The study group added caffeine citrate treatment on the basis of the control group,compared the clinical treatment of the two groups,before and after treatment,non-invasive ventilation,routine oxygen time,blood gas index and the days of apnea occurrence,and recorded whether there were complications during the treatment.Results The non-invasive ventilator assisted application time,oxygen days and clinical symptom relief time of children in the study group were shorter than those in the control group,and the improvement degree of arterial blood gas was higher than that in the control group(P<0.05);There was no significant difference in the total complication rate between the two groups(P>0.05).Conclusion The addition of caffeine citrate in the treatment of NRDS with non-invasive high-frequency concussion ventilation combined with PS can effectively shorten the days and oxygen time of noninvasive ventilator in children,improve their clinical symptoms faster,reduce the failure rate of withdrawal,and have high safety.
