分析内镜黏膜下剥离术治疗30例早期胃癌的临床疗效

Analysis of Clinical Effect of Endoscopic Submucosal Dissection in Treatment of 30 Cases Early Gastric Cancer

目的对早期胃癌患者采用黏膜下剥离术进行临床治疗进行回顾性分析与讨论。方法对于本院2020年1月进行早期胃癌治疗的30例患者采用内镜黏膜下剥离术进行临床手术治疗,从术前、术后患者的病情变化发展进行讨论与分析,并对其进行为期3年的随访工作,探讨术后胃癌复发率,对各项观察指标进行统计学分析。其中观察指标主要包括对并发症情况、血清因子水平、TGFβ1蛋白及幽门螺杆菌-lg G水平、复发情况以及生存质量进行数据统计分析,对其术后治疗情况及其早期胃癌的病情影响因素进行讨论与分析。结果针对30例早期胃癌患者进行内镜黏膜下剥离术,其术后并发症发生率仅为10%,且血清CA19-9、CEA、CA12-5、miR-125水平较术前显著降低(P<0.05)。TGFβ1蛋白、幽门螺杆菌-lgG水平也较术前显著降低(P<0.05)。在术后胃癌复发率的调查中,仅有2例患者于术后三年内复发胃癌,其复发率为6.67%,且术后患者生活质量较术前均得到了显著改善(P<0.05)。结论针对早期胃癌患者的病症对病灶进行内镜黏膜下剥离术的手术治疗,能够有效降低患者并发症的发生率,有助于进行长期治疗,提高预后治疗效率,具有良好治疗效果。

Objective To review and analyze clinical treatment of submucosal dissection for early gastric cancer patients.Methods The paper chose 30 patients with early gastric cancer treatment in our hospital in January 2020,treated with endoscopic submucosal dissection for clinical surgery,discussed and analyzed changes and development of condition of patients before and after surgery,with 3-year follow-up to investigate the recurrence rate of gastric cancer after surgery,carried on statistical analysis on various observation indicators,mainly including complications,serum factor levels,and TGFβ1 protein and helicobacter pylori lgG,recurrence,and life quality.Postoperative treatment status and influencing factors of early gastric cancer were discussed and analyzed.Results Endoscopic submucosal dissection was performed on 30 patients with early gastric cancer,incidence of postoperative complications was only 10%.Serum levels of CA19-9,CEA,CA12-5,and miR-125 were significantly lower than preoperative levels(P<0.05).Levels of TGFβ1 protein and helicobacter pylori lgG were significantly lower than before surgery(P<0.05).Investigation of postoperative gastric cancer recurrence rate showed,only 2 patients experienced gastric cancer recurrence within three years after surgery,with a recurrence rate of 6.67%,and life quality of postoperative patients was significantly better than before surgery(P<0.05).Conclusion Surgical treatment of lesions with endoscopic submucosal dissection can effectively reduce incidence of complications,facilitate long-term treatment,improve prognosis and treatment efficiency for early gastric cancer patients,which has good curative effect.
