主动脉内球囊反搏在复杂高危有指征患者病变介入治疗中的临床应用研究

Clinical study of intra-aortic balloon pump in interventional treatment of CHIP lesions

目的:观察主动脉内球囊反搏在复杂高危有指征患者(CHIP)病变介入治疗中的临床疗效。方法:选取62例CHIP病变介入治疗的患者资料进行回顾分析。将经皮冠状动脉介入治疗(PCI)术前保护性植入主动脉内球囊反搏的50例患者设为试验组,未植入主动脉内球囊反搏的12例患者设为对照组,比较两组术中及术后的临床情况。结果:试验组患者术中只有1例发生无复流,1例发生下肢血栓,对照组患者术中有2例发生急性左心衰,1例发生心脏骤停,1例发生无复流。术后试验组N端脑钠肽前体(NT-proBNP)水平较对照组显著降低,且心功能较对照组明显上升,差异均具有统计学意义(均P<0.05)。结论:主动脉内球囊反搏辅助支持下对CHIP病变介入治疗可提高患者的手术安全性,减少手术并发症,有效改善患者心功能。

Objective:To observe the clinical efficacy of intra-aortic balloon pump(IABP)in the interventional treatment of complex high-risk and indicated patients(CHIP)lesions.Methods:A retrospective analysis was conducted on the data of 62 patients with CHIP lesions undergoing interventional therapy.Fifty patients who received protective intra-aortic balloon pump before percutaneous coronary intervention(PCI)were assigned to the experimental group,while 12 patients who did not receive intra-aortic balloon pump were assigned to the control group.The clinical conditions during and after surgery were compared between the two groups.Results:Only one patient in the experimental group experienced no reflow during surgery,and one patient developed lower limb thrombosis.In the control group,two patients experienced acute left heart failure,one experienced cardiac arrest,and one experienced no reflow during surgery.The postoperative NT-proBNP levels in the experimental group were significantly lower than those in the control group,and cardiac function was significantly increased compared to the control group(all P<0.05).Conclusion:Interventional treatment of CHIP lesions with support of IABP can improve the surgical safety,reduce surgical complications,and effectively improve patient cardiac function.
