不同剂量沙库巴曲缬沙坦治疗慢性心力衰竭的短期疗效观察

Short-Term Effect Observation of Different Doses of Sarcubatrix Valsartan for Chronic Heart Failure

目的在慢性心力衰竭治疗中应用不同剂量沙库巴曲缬沙坦,探寻不同的短期疗效。方法将本院2022年1月—2023年3月收入的90例慢性心力衰竭患者纳进研究,患者均采用沙库巴曲缬沙坦治疗,后依照不同的给药剂量进行分组,分别为低剂量、中剂量和高剂量组,每组30例,其中低剂量组用药剂量为50mg,中剂量组用药剂量为100mg,高剂量组用药剂量为200mg。比较三组短期疗效。结果低剂量组治疗总有效率86.67%,中剂量组90.00%,高剂量组93.33%,相比较无差异(P>0.05);治疗后三组比较左室射血分数(LVEF)、6min步行距离水平(6MWT)高剂量组更高,左室舒张末内径(LVEDD)、左室收缩末内径(LVESD)高剂量组更低(P<0.05);治疗后高剂量组血浆NT-pro BNP比中剂量和低剂量组低(P<0.05);但是高剂量组用药不良反应总发生率较中剂量与低剂量组高(P<0.05)。结论慢性心力衰竭治疗中用200mg沙库巴曲缬沙坦治疗的优势更大,包括在心功能改良、减低血浆NT-pro BNP浓度等方面,但是因用药剂量较高,因此不良反应发生率也更高,因此临床在应用该药时尤其要注 意用药的剂量和安全性问题。

Objective To explore short term effect of different doses of sarcubatrix valsartan in treatment of chronic heart failure.Methods The paper chose 90 patients with chronic heart failure in the hospital from January 2022 to March 2023 for study,and divided them into low dose,medium dose,and high dose group according to different dosage levels,with 30 cases in each group.Low dose group was treated with dosage of 50mg,medium dose group with dosage of 100mg,and high dose group with dosage of 200mg.Short-term effect of three groups was compared.Total effective rate of low dose group was 86.67%,that of medium dose group was 90.00%,and that of high dose group was 93.33%,comparison showed no difference(P>0.05).After treatment,left ventricular ejection fraction(LVEF)and 6-minute walking distance level(6MWT)in high-dose group was higher among three groups,while left ventricular end diastolic diameter(LVEDD)and left ventricular end systolic diameter(LVESD)in high-dose group was lower(P<0.05).After treatment,plasma NT pro BNP in high-dose group was lower than medium and low dose groups(P<0.05).However,total incidence of adverse reactions in high-dose group was 26.67%,higher than 10.00% and 3.33% in medium and low dose groups(P<0.05).Conclusion 200mg of sarkubactrian valsartan can achieve more advantages for chronic heart failure,including improving cardiac function and reducing plasMA NT pro BNP concentration.However,due to higher dosage,incidence of adverse reactions is higher.Therefore,it is particularly important to pay attention to dosage and safety of medication in clinical practice.
