摘要
目的探讨肺部超声(LUS)联合炎症指标血清降钙素原(PCT)、C反应蛋白(CRP)对无咳嗽症状小儿肺炎的诊断价值。方法选取2022年1月—2023年3月于本院儿科就诊的无咳嗽症状的感染性肺炎患儿55例,再选取同期非肺炎的患儿39例作为对照组,对比两者的LUS、CRP及PCT的差异,并检测肺炎组LUS、PCT、CRP及联合检测的敏感性、特异性及描绘ROC曲线。结果肺炎组的小儿LUS评分及PCT、CRP水平均高于对照组(P<0.05);肺炎组的LUS评分、PCT及CRP的曲线下面积(AUC)分别约0.967、0.772、0.793,联合诊断AUC提升至0.983。结论LUS、CRP及PCT对细菌性新生儿肺炎的诊断均有较好的相关性,联合诊断能提高其诊断效果。
Objective To explore diagnostic value of lung ultrasound(LUS)combined with inflammatory markers of serum procalcitonin(PCT)and C-reactive protein(CRP)for children with pneumonia without cough symptoms.Methods The paper chose 55 children with infectious pneumonia without cough symptoms in our pediatric department from January 2022 to March 2023,and chose 39 children without pneumonia during the same period as control group.Difference of LUS,CRP,and PCT between two groups was compared,and sensitivity,specificity,and ROC curve of LUS,PCT,CRP and combined detection was detected.Results LUS scores,PCT,and CRP levels of children in pneumonia group were higher than control group(P<0.05).LUS scores,area under curve(AUC)of PCT,and CRP in pneumonia group were approximately 0.967,0.772,and 0.793 respectively,combined diagnosis AUC increased to 0.983.Conclusion LUS,CRP,and PCT can achieve good correlation in diagnosis of bacterial neonatal pneumonia,and combined diagnosis can improve the diagnostic effect.
