颈后路单开门椎板固定板系统椎管扩大成形术在颈椎手术的临床研究

Clinical Study of Posterior Cervical Single Open Door Spinal Plate Fixation System for Cervical Surgery Based on Laminoplasty

目的分析针对颈椎手术患者,在行椎管扩大成形术基础上,应用颈后路单开门椎板内固定板系统的临床价值。方法抽取2020年1月—2022年9月于本院就诊的多节段脊髓型颈椎病和过伸伤脊髓损伤患者,共计40例。以治疗方式为分组依据分设参比组(保守治疗,n=20)与分析组(颈后路单开门椎板扩大成形术,n=20),比较两组治疗前后ASIA运动评分、ASIA感觉评分、神经功能评分、疼痛评分、颈椎曲度、颈椎活动度、生活质量、住院时间等指标,并记录分析组手术时间、术中出血时间。结果分析组治疗后ASIA运动评分、ASIA感觉评分、JOA运动评分、颈椎曲度及颈椎活动度显著高于参比组及两组治疗前;分析组治疗后VAS评分显著低于参比组及两组治疗前,数据比较有意义(P<0.05);分析组治疗后生活质量评分高于参比组,住院时间短于参比组,数据比较有意义(P<0.05)。结论针对颈椎手术患者,在行椎管扩大成形术基础上,应用颈后路单开门椎板内固定板系统治疗,能够改善患者颈椎关节功能,缩短住院时间,提升术后生活质量,可推广。

Objective To analyze clinical value of posterior cervical single door laminoplasty system for cervical spine surgery patients based on laminoplasty.Methods The paper chose 40 patients with multi-segment cervical spondylotic myelopathy and spinal cord injury in our hospital from January 2021 to September 2022,and divided them into two groups based on treatment method,reference group(conservative treatment,n=20)and analysis group(posterior cervical single door laminoplasty,n=20).ASIA motor scores,ASIA sensory scores,neurological function scores,pain scores,cervical curvature,cervical mobility,life quality,hospital stay,and other indicators were compared before and after treatment between two groups,and surgical time and intraoperative bleeding time between two groups was recorded and analyzed.Results After treatment,ASIA motor scores,ASIA sensory scores,JOA motor scores,cervical curvature,and cervical mobility in analysis group were significantly higher than control group and before treatment.VAS scores of analysis group after treatment were significantly lower than reference group before treatment,data were statistically significant(P<0.05).Life quality scores of analysis group after treatment were higher than reference group,length of stay was shorter than reference group,data were statistically significant(P<0.05).Surgical time of analysis group was(130.41±23.25)minutes,and intraoperative blood transfusion volume was(336.12±28.25)mL.Conclusion Posterior cervical single door laminoplasty system based on laminoplasty can improve cervical joint function,shorten hospital stay,and improve postoperative life quality of cervical spine surgery patients,which can be promoted.
