腹腔镜联合支架治疗左半结肠癌伴梗阻疗效及对患者应激反应和免疫功能影响

Effect of Laparoscopy Combined with Stent in the Treatment of Left Colon Cancer with Obstruction and Its Influence on Stress Response and Immune Function of Patients

目的:研究腹腔镜联合支架治疗左半结肠癌伴梗阻疗效及对患者应激反应和免疫功能的影响。方法:将46例左半结肠癌伴梗阻患者根据术式不同分为对照组和观察组,分别采用开腹手术和腹腔镜联合支架进行治疗。观察两组手术及术后情况、并发症情况、手术前后的应激反应和免疫情况。结果:观察组术中出血量显著低于对照组(P<0.05),手术时间高于对照组,但组间无统计学差异(P>0.05);抗生素使用时间、术后肛门排气时间、住院时间均显著低于对照组(P<0.05)。观察组并发症发生率显著低于对照组(P<0.05)。术前两组CRP、CD3^(+)和CD4^(+)/CD8^(+)差异均无统计学意义(P>0.05),术后两组CRP、CD3^(+)和CD4^(+)/CD8^(+)差异均具有统计学意义(P<0.05)。结论:腹腔镜联合支架治疗左半结肠癌伴梗阻疗效显著。

Objective:To study the effect of laparoscopy combined with stent in the treatment of left colon cancer with obstruction and its influence on the stress response and immune function of patients.Methods:46 cases of left colon cancer with obstruction were divided into control group and observation group according to different operation methods,and were treated with laparotomy and laparoscopy combined with stent respectively.The operation and postoperative conditions,complications,stress reaction and immunity before and after operation were observed in the two groups.Results:The amount of intraoperative bleeding in the observation group was significantly lower than that in the control group(P<0.05),and the operation time was higher than that in the control group,but there was no statistical difference between the two groups(P>0.05).The use time of antibiotics,postoperative anal exhaust time,and hospitalization time were significantly lower than those in the control group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).There was no significant difference in CRP,CD3^(+)and CD4^(+)/CD8^(+)between the two groups before operation(P>0.05),there was statistically significant difference in CRP,CD3^(+)and CD4^(+)/CD8^(+)between the two groups after operation(P<0.05).Conclusion:Laparoscopy combined with stent is effective in the treatment of left colon cancer with obstruction.
