西格列汀联合门冬胰岛素治疗老年2型糖尿病患者的疗效及对脂代谢、炎症因子、同型半胱氨酸的影响

Effect of Sitagliptin Combined with Insulin Aspartate on Lipid Metabolism,Inflammatory Factors and Homocysteine in Elderly Patients with Type 2 Diabetes Mellitus

目的 分析老年2型糖尿病患者采用西格列汀联合门冬胰岛素治疗的效果。方法 选取2021年3月—2022年8月南京市高淳人民医院收治的老年2型糖尿病患者70例为研究对象,按随机数表法分成两组,各35例。对照组给予门冬胰岛素治疗,观察组给予西格列汀联合门冬胰岛素治疗,比较两组临床疗效。结果 观察组治疗总有效率为94.29%,高于对照组的77.14%,差异有统计学意义(χ^(2)=4.200, P<0.05);治疗后,观察组总胆固醇(totalcholesterol,TC)、三酰甘油(triacylglyceride,TG)、低密度脂蛋白胆固醇(lowdensity lipoprotein-cholesterol, LDL-C)、同型半胱氨酸(homocysteine, Hcy)水平均低于对照组,高密度脂蛋白胆固醇(high density lipoprotein-cholesterol, HDL-C)水平高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组白细胞介素(interleukin, IL)-6、C反应蛋白(C-reactive protein, CRP)、IL-8水平均低于对照组,差异有统计学意义(P<0.05)。结论 西格列汀联合门冬胰岛素治疗老年2型糖尿病患者疗效显著,能调节脂代谢,降低Hcy及炎症因子水平。

Objective To analyze the efficacy of sitagliptin combined with insulin aspartate in elderly patients with type 2 diabetes.Methods A total of 70 elderly patients with type 2 diabetes admitted to Nanjing Gaochun People's Hospital from March 2021 to August 2022 were selected as the study objects and were divided into two groups according to random number table method,with 35 cases in each group.The control group was treated with insulin aspartate,and the observation group was treated with sitagliptin combined with insulin aspartate.The clinical efficacy of the two groups was compared.Results The total effective rate of observation group was 94.29%,which was higher than 77.14%of control group,the difference was statistically significant(χ^(2)=4.200,P<0.05).After treatment,the values of total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and homocysteine(Hcy)in the observation group were lower than those in the control group,but the high density lipoprotein cholesterol(HDL-C)was higher than that in control group,the differences were statistically significant(P<0.05).After treatment,the values of interleukin(IL)-6,C-reactive protein(CRP)and IL-8 in observation group were lower than those in control group,the differences were statistically significant(P<0.05).Conclusion Sitagliptin combined with insulin aspartate is effective in the treatment of elderly patients with type 2 diabetes,which can regulate lipid metabolism,reduce the level of Hcy and inflammatory factors.
