初发1型糖尿病酮症酸中毒中糖化血红蛋白、心肌损伤标志物研究

摘要 目的 探讨1型糖尿病(type 1 diabetes mellitus, T1DM)患儿在诊断时是否存在酮症酸中毒(diabetic ketoacidosis, DKA)以及糖化血红蛋白(glycated hemoglobin, HbA1c)水平和心肌损伤标志物。方法 选取2020年1月—2023年1月泉州市第一医院收治的68例T1DM患儿为研究对象,根据是否存在DKA将其分为研究组(存在DKA,53例)和对照1组(未存在DKA,15例),同时选择68例健康人群作为对照2组。收集患儿的基线特征,包括HbA1c水平和心肌损伤标志物。结果 研究组的血红蛋白水平较对照1组较高,差异有统计学意义(P<0.05)。研究组心肌肌钙蛋白、肌酸激酶同工酶和肌酸激酶水平均高于对照1组,差异有统计学意义(P<0.05)。结论 初发1型糖尿病酸中毒患儿的糖化血红蛋白水平与心肌损伤标志物水平较高。因此,在T1DM患者中预防和治疗DKA以及改善血糖控制是十分重要的。 Objective To investigate the presence of diabetic ketoacidosis(DKA)in children with type 1 diabetes mellitus(T1DM)at the time of diagnosis,as well as glycated hemoglobin(HbA1c)levels and myocardial injury markers.Methods 68 T1DM children admitted to Quanzhou First Hospital from January 2020 to January 2023 were selected as the study subjects.They were divided into a study group(53 with DKA)and a control group 1(15 without DKA)based on the presence of DKA.Additionally,68 healthy individuals were selected as the control group 2.Collected baseline characteristics of children,including HbA1c levels and myocardial injury markers.Results The hemoglobin levels in the study group was higher than that in the control group 1,and the difference was statistically significant(P<0.05).The levels of cardiac troponin,creatine kinase isoenzyme,and creatine kinase in the study group were higher than those in the control group 1,and the differences were statistically significant(P<0.05).Conclusion The level of glycosylated hemoglobin and myocardial injury markers in patients with acidosis of type 1 diabetes mellitus were higher.Therefore,it is crucial to prevent and treat DKA and improve blood glucose control in T1DM children.
