经阴道三维超声容积成像联合经腹超声对子宫肌瘤分型的诊断价值

The Diagnostic Value of Transvaginal Three-Dimensional Ultrasound Volumetric Imaging Combined with Transabdominal Ultrasound in the Classification of Uterine Fibroids

目的探讨经阴道三维超声容积成像联合经腹超声对子宫肌瘤分型的诊断价值。方法回顾性分析2020年1月至2022年12月医院收治的78例子宫肌瘤患者的临床资料,所有患者均行经阴道三维超声容积成像、经腹超声检查。以手术病理分型结果为金标准,分析经阴道三维超声容积成像、经腹超声及两者联合对子宫肌瘤分型的诊断价值。结果手术病理分型结果显示,78例子宫肌瘤患者中0、1、2、3、4、5、6、7、2~5型分别为2、5、4、15、32、7、6、2、5例。Kappa一致性检验结果显示,经阴道三维超声容积成像、经腹超声与手术病理分型结果一致性较强(Kappa=0.800、0.685,P<0.001);经阴道三维超声容积成像联合经腹超声与手术病理分型结果一致性强(Kappa=0.967,P<0.001)。经阴道三维超声容积成像联合经腹超声对子宫肌瘤分型诊断的准确度高于经阴道三维超声容积成像、经腹超声,差异有统计学意义(P<0.05)。结论经阴道三维超声容积成像联合经腹超声对子宫肌瘤分型的诊断价值较高,可为临床诊断、治疗该疾病患者提供科学的参考依据。

Objective To explore the diagnostic value of transvaginal threedimensional ultrasound volumetric imaging combined with transabdominal ultrasound in the classification of uterine fibroids.Methods With the retrospective analysis of clinical data of 78 patients with uterine fibroids admitted to the hospital from January 2020 to December 2022,all patients underwent transvaginal three-dimensional ultrasound volumetric imaging and transabdominal ultrasound examination.With results of surgical pathological classification as the"gold standard",the diagnostic value of transvaginal three-dimensional ultrasound volumetric imaging,transabdominal ultrasound,and the combination of the two methods in the classification of uterine fibroids was analyzed.Results The surgical pathological classification results showed that among 78 patients with uterine fibroids,there were 2,5,4,15,32,7,6,2,and 5 cases with 0,1,2,3,4,5,6,7 and 2-5 types,respectively.The Kappa consistency test results showed a strong consistency between transvaginal three-dimensional ultrasound volumetric imaging,transabdominal ultrasound,and surgical pathological classification results(Kappa=0.800,0.685,P<0.001);The combination of transvaginal three-dimensional ultrasound volume imaging and transabdominal ultrasound showed strong consistency with surgical pathological classification results(Kappa=0.967,P<0.001).The accuracy of transvaginal three-dimensional ultrasound volumetric imaging combined with transabdominal ultrasound in the classification diagnosis of uterine fibroids was higher than that of transvaginal three-dimensional ultrasound volumetric imaging and transabdominal ultrasound,and the differences were statistically significant(P<0.05).Conclusion With high diagnostic value in the classification of uterine fibroids,the combination of transvaginal three-dimensional ultrasound volumetric imaging and transabdominal ultrasound can provide scientific reference basis for clinical diagnosis and treatment of patients with this disease.
