肺炎支原体肺炎患儿血清嗜酸细胞趋化因子、IL-25及IL-33与肺功能的相关性探讨

摘要 目的探讨肺炎支原体肺炎患儿肺功能与血清嗜酸细胞趋化因子(Eotaxin)及白细胞介素(IL)-25、IL-33的相关性。方法回顾性选取2020年2月至2023年2月广安市人民医院肺炎支原体肺炎患儿60例作为肺炎组,60名健康体检儿童作为健康组。统计分析两组血清学指标[肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-4、IL-6、IL-25、IL-33、Eotaxin、嗜酸性粒细胞阳离子蛋白(ECP)、半胱氨酰白三烯(CysLTs)]、肺功能指标[最大呼气峰流速值(PEF)、最大深呼吸后第1秒呼出气量容积(FEV1)、用力肺活量(FVC)、呼出75%潮气量时的瞬间流速/潮气呼气峰流速比(TEF25/PTEF%)、达峰时间比(tPTEF/TE)、潮气量],分析肺炎组血清Eotaxin与IL-25、IL-33的相关性、肺功能与血清Eotaxin及IL-25,IL-33的相关性。结果肺炎组患儿的血清TNF-α、IL-4、IL-6、IL-25、IL-33、Eotaxin、ECP、CysLTs水平分别为(3.06±0.93)mg/L、(42.76±7.61)ng/L、(58.63±9.53)pg/mL、(46.15±7.27)ng/L、(11.64±2.34)ng/L、(111.51±19.04)ng/L、(29.04±4.52)μg/L、(638.40±99.50)ng/L,均明显高于健康组[(0.91±0.30)mg/L、(26.44±4.52)ng/L、(24.57±4.61)pg/mL、(39.78±6.85)ng/L、(10.34±1.01)ng/L、(102.84±19.53)ng/L、(14.86±2.52)μg/L、(492.70±79.63)ng/L],差异均有统计学意义(P<0.05)。肺炎组患儿的PEF、FEV1、FVC、TEF25/PTEF%、tPTEF/TE、潮气量分别为(73.20±9.78)%、(69.88±9.41)%、(66.63±10.11)%、(66.10±11.41)%、(36.20±6.68)%、(7.94±1.21)mL/kg,均明显低于健康组[(88.50±9.61)%、(85.55±9.16)%、(87.20±11.53)%、(78.55±14.84)%、(45.96±7.01)%、(9.23±1.37)mL/kg],差异均有统计学意义(P<0.05)。肺炎组患儿的血清Eotaxin与IL-25、IL-33均呈显著正相关(P<0.05)。肺炎组患儿的PEF、FEV1、FVC、TEF25/PTEF%、tPTEF/TE、潮气量与血清Eotaxin及IL-25、IL-33均呈显著负相关(P<0.05)。结论肺炎支原体肺炎患儿清Eotaxin与IL-25、IL-33水平均呈显著正相关,而肺功能与血清Eotaxin及IL-25、IL-33均呈显著负相关。通过检测肺炎支原体肺炎患儿血清Eotaxin与IL-25、IL-33水平可为临床诊断与病情评估提供有效依据。 Objective To investigate the correlation between lung function and serum eosinophilic chemotactic factor(Eotaxin),interleukin(IL)-25 and IL-33 in children with mycoplasma pneumoniae pneumonia.Methods From February 2020 to February 2023,60 cases of children with mycoplasma pneumoniae pneumonia in Guang'an People's Hospital were retrospectively selected as the pneumonia group and 60 cases of healthy children undergoing physical examination as the healthy group.Serological indexes[tumor necrosis factor alpha(TNF-α),IL-4,IL-6,IL-25,IL-33,Eotaxin,eosinophil cationic protein(ECP),cysteinyl leukotriene(CysLTs)]and pulmonary function indexes[peak expiratory flow(PEF),forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC),flow rate of exhaling 75%tidal volume/peak tidal expiratory flow(TEF25/PTEF%),peak time ratio(tPTEF/TE),tidal volume]of the two groups were statistically analyzed,and the correlation between serum Eotaxin and IL-25 and IL-33,lung function and serum Eotaxin and IL-25 and IL-33 in the pneumonia group were analyzed.Results The levels of serum TNF-α,IL-4,IL-6,IL-25,IL-33,Eotaxin,ECP,and CysLTs in the pneumonia group were(3.06±0.93)mg/L,(42.76±7.61)ng/L,(58.63±9.53)pg/mL,(46.15±7.27)ng/L,(11.64±2.34)ng/L,(111.51±19.04)ng/L,and(29.04±4.52)μg/L,(638.40±99.50)ng/L,respectively,which were significantly higher than those in the healthy group[0.91±0.30)mg/L,(26.44±4.52)ng/L,(24.57±4.61)pg/mL,(39.78±6.85)ng/L,(10.34±1.01)ng/L,(102.84±19.53)ng/L,(14.86±2.52)μg/L,(492.70±79.63)ng/L,the differences were statistically significant(P<0.05).The PEF,FEV1,FVC,TEF25/PTEF%,tPTEF/TE,and tidal volume of children in the pneumonia group were(73.20±9.78)%,(69.88±9.41)%,(66.63±10.11)%,(66.10±11.41)%,(36.20±6.68)%,and(7.94±1.21)mL/kg,respectively,which were significantly lower than those in the healthy group[(88.50±9.61)%,(85.55±9.16)%,(87.20±11.53)%,(78.55±14.84)%,(45.96±7.01)%,(9.23±1.37)mL/kg],the differences were statistically significant(P<0.05).Serum Eotaxin was significantly positively correlated with IL-25 and IL-33 in the pneumonia group(P<0.05).PEF,FEV1,FVC,TEF25/PTEF%,tPTEF/TE,tidal volume were significantly negatively correlated with serum Eotaxin,IL-25 and IL-33 in the pneumonia group(P<0.05).Conclusion There is a significant positive correlation between serum Eotaxin,IL-25,and IL-33 levels in children with Mycoplasma pneumoniae pneumonia,while there is a significant negative correlation between lung function and serum Eotaxin,IL-25,and IL-33 levels.The detection of serum Eotaxin,IL-25,and IL-33 levels in children with Mycoplasma pneumoniae pneumonia can provide effective basis for clinical diagnosis and disease assessment.
