摘要
目的通过分析平顶山市2004—2022年人类免疫缺陷病毒(HIV)/艾滋病(AIDS)患者抗病毒治疗情况,评估抗病毒治疗效果,为本区域的艾滋病防治工作提供科学依据。方法使用“中国疾病预防控制信息系统”,下载2004—2022年现地址在平顶山市,抗病毒治疗满6个月及以上的HIV/AIDS病例数据资料,检测CD4^(+)T淋巴细胞和病毒载量,分别作为抗病毒治疗效果的免疫学和病毒学评价指标。对数据统计分析采用SPSS 22.0。结果共纳入2186例患者,治疗前基线CD4^(+)T细胞计数均值为(286.32±189.81)个/μl,抗病毒治疗后最近一次随访检测CD4^(+)T细胞计数均值为(446.45±221.96)个/μl,抗病毒治疗前后CD4^(+)T细胞结果分布差异有统计学意义(χ^(2)=641.53,P<0.001)。患者最近一次随访检测VL<20 copies/ml有1875例(85.77%),构成比呈逐渐上升趋势(χ^(2)=16.970,P<0.001)。Spearman检验抗病毒治疗后CD4^(+)T细胞与VL结果呈负相关(r=-0.143,P<0.001)。结论平顶山市在治的抗病毒治疗HIV/AIDS患者,免疫重建和病毒抑制情况较好,应继续扩大检测、早发现、早治疗,加强患者服药依从性教育,坚持长期规范治疗,进一步提高抗病毒治疗质量。
Objective To provide scientific basis for HIV/AIDS prevention and treatment in Pingdingshan by analyzing the status of antiretroviral therapy among HIV/AIDS patients from 2004 to 2022 and evaluating the effect of antiretroviral therapy.Methods To download data on HIV/AIDS patients who have been receiving antiviral treatment for more than 6 months current address in Pingdingshan City from 2004 to 2022,through the“China Disease Prevention and Control Information System”.CD4^(+)T lymphocytes and viral load were detected as immunological and virological evaluation indicators of antiviral treatment effect respectively.SPSS 22.0 was applied for statistical analysis of the data.Results A total of 2186 patients were enrolled which the baseline CD4^(+)T lymphocyte count was(286.32±189.81)/μl before treatment.The mean CD4^(+)T lymphocyte count was(446.45±221.96)/μl during the last follow-up after antiviral therapy.There was significant difference in the distribution of CD4^(+)T lymphocyte before and after antiviral treatment(χ^(2)=641.53,P<0.001).There were 1875(85.77%)patients with VL<20 copies/ml at the last follow-up,and the constituent ratio was gradually increasing(χ^(2)=16.970,P<0.001).Spearman test showed a negative correlation between CD4^(+)T lymphocyte and VL(r=-0.143,P<0.001).Conclusion The antiviral treatment HIV/AIDS patients under treatment in Pingdingshan have good immune reconstitution and virus suppression.We should continue to expand detection,early detection and early treatment,strengthen patient compliance education,adhere to long-term standardized treatment,and further improve the quality of antiviral treatment.
