角膜缘干细胞移植术联合翼状胬肉切除术对翼状胬肉患者的临床效果

Clinical effect of limbal stem cell transplantation combined with pterygium excision on patients with pterygium

目的探讨临床治疗翼状胬肉疾病采用角膜缘干细胞移植术、翼状胬肉切除术联合方式的效果。方法选择2016年3月1日至2019年9月30日常州市第七人民医院眼科收诊治疗的翼状胬肉患者98例进行研究,按照随机数表法分为两组,对照组(49例)单纯采用翼状胬肉切除术治疗;观察组(49例)在对照组基础上联合角膜缘干细胞移植术治疗。比较两组治疗效果,分别记录各组患者治疗所用时间,比较组间差异性。观察两组患者术后产生的并发症表现,统计比较总发生率。结果观察组翼状胬肉患者经联合术式治疗后,总有效率明显高于对照组,疾病复发率低于对照组,差异有统计学意义(P<0.05)。观察组创口愈合及住院治疗总时间均低于对照组,差异有统计学意义(P<0.05)。观察组术后产生的创面水肿等并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论临床治疗翼状胬肉可在翼状胬肉切除术基础上联用角膜缘干细胞移植术式,相比单纯切除治疗效果更佳,术后患者复发率更低且并发症少,值得运用推广。

Objective To explore the effect of the combination of limbal stem cell transplantation and pterygium excision in the clinical treatment of pterygium diseases.Methods A study was conducted on 98 patients with pterygium admitted to the Ophthalmology Department of the Seventh People’s Hospital of Changzhou from March 1,2016 to September 30,2019.They were divided into two groups according to the random number table method.The control group(49 cases)was treated solely with pterygium excision.The observation group(49 cases)was additionally treated with limbal stem cell transplantation based on the treatment in the control group;The treatment effects were compared,the time spent on treatment for each group of patients was recorded,and the differences between the groups were compared.The postoperative complications of the two groups of patients were observed and the total incidence rate was statistically compared.Results The total effective rate of patients with pterygium in the observation group after combined surgical treatment was significantly higher than that in the control group,and the disease recurrence rate was lower than that in the control group,with statistically significant differences(P<0.05).The total time of wound healing and hospitalization in the observation group was shorter than that in the control group,with statistically significant difference(P<0.05).The total probability of postoperative complications such as wound edema in the observation group was lower than that in the control group,with statistically significant difference(P<0.05).Conclusion The clinical treatment of pterygium can be conducted through the combination of limbal stem cell transplantation and pterygium excision,which is more effective compared with simple excision,with a lower recurrence rate and fewer complications for postoperative patients.Therefore,it is worth promoting and applying.
